Survival Analysis About Arognosis Of Patients With Colorectal Cancer

Objective: Research under the survival condition of postoperative patients with colorectal cancer in the influence of different risk factors.Methods: We retrospectively analyzed clinical and pathological data of206 patients with colorectal cancer in the Fourth Affiliated Hospital Of Hebei Medical University between January 2009 and January 2010.Kaplan-Meier method was used for survival analysis.The patients are followed by telephone or letter.The study starts with operation time,to patients died,lost or the latest follow-up time for the node.The end of the study is death.Log-rank test was applied for univariate analysis.Cox proportional hazards models was used in the multiple factors regression analysis.Results:1 206 patients are collected in this study,among which,there are 126 male patients(61.2%)and 80 female patients(38.8%).The youngest age is 22 years old,the oldest age is 84 years old age,,and the median age is 62 years old.2 Univariate analysis2.1 gender,age(< 65 years old,65 or higher),tumor size(<5cm,5cm or higher)and tumor location(colon,rectum)had no significant effect on median survival time,3-years or 5-year survival rate of postoperative patients,(P >0.05).2.2 Patients without preoperative intestinal obstruction had better median survival time(76.63 months vs 50.57 months),3-years survival rate(79.0% vs54.5%)and 5-year survival rate(71.8% vs 45.5%)than their counterparts with preoperative intestinal obstruction.Median survival time of high or middle differentiation group and low differentiation group was 77.60 months and60.13 months;3-year survival rate was 80.8% and 61.8%;5-year survival rate was 73.8% and 52.9%.The difference is statistically significant,P=0.012.Median survival time,3-years survival rate,5-year survival rate of patients without lymphatic metastasis was 77.83 months,87.0%,82.1%;Median survival time,3-years survival rate,5-year survival rate of patients with lymphatic metastasis was 63.47 months,63.9%,53.0%.The difference is statistically significant,P<0.001.Median survival time,3-years survival rate,5-year survival rate of patients without distant metastasis was 76.63 months,80.3%,73.0%.Median survival time,3-years survival rate,5-year survival rate of patients with distant metastasis was 20.50 months,23.1%,11.5%.The difference is statistically significant,P=0.008.Median survival time of different T stage(T1-T4)was 83.64 months,80.10 months,78.03 months and 74.17 months,3-year survival rate was 100%,92.6%,88.7% and69.4%;5-year survival rate was 100%,88.5%,84.9% and 59.5%.The difference is statistically significant,P=0.003.Median survival time of different tumor stage(?-?)was 79.00 months,77.67 months,65.33 months and 20.50 months,3-year survival rate was 89.7%,87.9%,67.1% and 23.1%,5-year survival rate was 86.2%,81.3%,56.2% and 11.5%.The difference is statistically significant,P<0.001.3 Multivariate analysis showed that preoperative intestinal obstruction(P=0.038),tumor stage(P=0.016),tumor differentiation degree(P=0.003),T stage(P=0.016),M stage(P=0.025)were independent risk factors to affect the postoperative survival condition of patients with colorectal cancer.Conclusions:1 Preoperative intestinal obstruction,tumor stage,tumor differentiation degree,T stage,lymphatic metastasis,and M stage have a significant effect on median survival time,3-years survival rate and 5-years survival rate.2 Preoperative intestinal obstruction,tumor stage,tumor differentiation degree,T stage and M stage were independent risk factors affecting survival condition of postoperative patients.
