Objective:To investigate the value of the total hemoglobin concentration(HbT)through ultrasound-diffuse optical tomography(US-DOT)in evaluation of the response to neoadjuvant chemotherapy(NCT)in patients with breast cancer,and the relationship between different molecular classification of breast cancer.Methods:(1)Fifth-two women with breast cancer confirmed by biopsy-pathology scheduled to receive NCT were recruited.(2)The tumor diameter size and total hemoglobin concentration(HbT)was assessed using ultrasound-diffuse optical tomography(US-DOT)measurements,acquired before neoadjuvant chemotherapy and operative treatment.Tumor pathologic variables of tumor type acquired before NAC in biopsy specimens,and the postoperative pathological information were also used in this research.Collect the pathology immunohistochemical data of all the cases before the NCT pathology immunohistochemical data,analyse and compare the differences of HbT in different molecular types before and after neoadjuvant chemotherapy.(3)According to the response to treatment in solid tumors,patients were divided in to different curative effect group.The patients' pathologic response was graded based on the Miller-Payne system.The analysis of variance compared the differences of HbT before and after neoadjuvant chemotherapy in each group.Pearson Correlation analyzed the relation ship between ther elative HbT variation and the tumor size changes.(3)Spearman's rank correlation coefficient or Spearman's ?,was computed between each tumor's Miller-Payne grade and the pretreatment total hemoglobin concentration.Results:(1)There were no significant differences in HbT between the CR,PR and SD groups before neoadjuvant chemotherapy(P ? 0.05),and that were significant differences in HbT between CR,PR and SD groups after neoadjuvant chemotherapy.After neoadjuvant chemotherapy,CR,PR group was significantly lower than before(P?0.05),but SD group was no significant difference about it(P =0.149).Analysis the change ratio of HbT in different groups through neoadjuvant chemotherapy : CR> PR> SD.(2)HbT of the effective group was significantly lower than that before neoadjuvant chemotherapy(P <0.01).There was no significant difference in HbT in the ineffective group(MP = 2-5)(P =0.86).(3)The change of HbT after neoadjuvant chemotherapy showed positive correlation with the change in tumor size(r =0.59).And the change of HbT after neoadjuvant chemotherapy showed positive correlation with the change in the grades of Miller-Payne(?=0.64).(4)After neoadjuvant chemotherapy,the three groups of different molecular pathology classification was significantly lower than before(P?0.05).The HbT,and the chance of HbT were no significant differences in the three groups of different molecular pathology classification before and after neoadjuvant chemotherapy(P?0.05).Conclusions:(1)After neoadjuvant chemotherapy the tumor HbT are generally decreases and the change of HbT showed positive correlation with the change in tumor size and the grades of Miller-Payne.That is to say,the more tumor HbT falling,neoadjuvant chemotherapy curative effect is better.(2)These initial findings indicate that hemoglobin parameters determined by US-DOT may provide a effective tool for predicting the response to neoadjuvant chemotherapy.(3)The HbT had no relation to breast cancer molecular classification.
