Objective:To explore the application value of black blood MRI in Shamblin typing of carotid body tumor;To discuss the difference of the incidence of plaque in carotid body tumor side and and healthy side,and determine the nature of the plaque;So as to provide an important reference for the clinical treatment of CBT.Methods:Collected 21 cases of carotid body tumor treated by cardiothoracic surgery in our hospital from December 2014 to December 2016,all patients were scanned with black blood MRI before operation.The tumor was divided into I,II,III typing according to the maximum wrapping degree of carotid body tumor,and compared with the results of surgery;then further analyze the relationship between the type and the amount of intraoperative blood loss.The number of carotid body tumor and contralateral side with plaques were recorded simultaneously,then evaluate the plaque stability based on signal characteristics.Results:1.14,7,0 of 21 carotid body tumor were classified as shamblin I,II,and III type,respectively.Compared with the surgical and pathological findings,the accuracy up to 100%.2.The average intraoperative blood loss of Shambin I carotid body tumor was 261.43 ml,Shambin II was 571.43 ml,and there was significant difference between different types and blood loss(P= 0.021).3.There was no significant difference in the incidence of plaque between the tumor side and the non tumor side,but the probability of the occurrence of the tumor side plaque was significantly higher than that of the non tumor side(P = 0.19).4.There were 17 plaques in this group,mostly in the carotid bifurcation,calcified plaque 8,lipid core 1,fibrous cap 1,early 8 plaque.Conclusion:1.Black blood MRI has a higher value in preoperative Shamblin typing of carotid body tumor,and then provide an accurate basis for the formulation of clinical operation plan.2.The classification of carotid body tumor is closely related to the amount of intraoperative bleeding,the higher the level,the greater the amount of bleeding.3.Plaque is more likely to occur on the side of carotid body tumor,which may be related to the changes of the anatomic structure of the carotid artery.4.For patients with plaques,black blood MRI can assess effectively the stability of plaque according to the signal characteristics before surgery.
