The Values Of Quantitative Analysis Of Magnetic Resonance ADC Value In The Diagnosis Of Prostate Cancer And The Classification Of Gleason Score

Objective: To evaluate the ADC quantitative analysis of magnetic resonance imaging in the diagnosis and differential diagnosis of prostate cancer,while to explore the correlation between ADC value and Gleason score of prostate cancer,and to evaluate the value of ADC value in the classification of prostate cancer,selection of treatment regimen and prognosis.Materials and Methods: To retrospective analysis of 89 cases of prostate cases which were confirmed by Ultrasound-guided biopsy or surgical pathology of Qingdao university hospital from 2010 to 2013.According to the pathology of prostate,the prostate lesion was divided into prostate cancer group,the benign prostate hyperplasia and the normal tissue group.Record the ADC values of each prostate lesion from mri examination and the Gleason score of each prostate cancer lesion.To analysis the differences of ADC value between prostate cancer,benign prostatic hyperplasia and normal prostate tissue,at the same time,take the ROC curve analysis of prostate cancer was diagnosed by ADC value.To analysis the correlation between ADC value and Gleason score of prostate cancer and the difference of ADC values among each score group,at the same time,take the ROC curve analysis of prostate cancer in high-risk groups was diagnosed by ADC value.Results: We have received 543 effective ROIs from 89 prostate cases,which were 224 prostate cancer tissues,276 benign prostatic hyperplasia tissues and 43 normal prostate tissues.There was a significant difference in ADC values between prostate cancer tissue and non-cancer tisue?U=66250.50,p<0.01?.There was a significant difference between group?F = 382.359,P < 0.01?.According to multiple comparison,the ADC values of prostate cancer group have a significant difference with prostatic hyperplasia and normal group?P < 0.05?.There was no statistical difference between prostate hyperplasia and normal tissue ADC?P > 0.05?.According to the ROC curve,when the cut-off value takes?1.08 × 10^-3mm² / s can be obtained with higher diagnostic performance which specificity and sensitivity were 84.8% and 95.9%.The ADC value and Gleason score of prostate cancer was negatively correlated?r =-0.439,P<0.01?.There was significant statistically difference?p<0.01?between Gleason score ?7 points groups and Gleason score =6 points groups and there was no statistically difference between Gleason score = 7 points,8 points and 9 points groups through multiple comparisons.According to the ROC curve,when the cut-off value takes?0.997 × 10^-3mm² / s can be obtained with higher diagnostic performance to distinguish middle-high risk group?Gleason score ?7 points?and low risk group?Gleason score <7?of prostate cancer which specificity and sensitivity were 96.2% and 60%.Conclusion: The DWI ADC and quantitative analysis of ADC value of magnetic resonance imaging can be used to identify prostate cancer,benign prostatic hyperplasia and normal prostate tissue.In the diagnosis and differential diagnosis of prostate disease the ADC value had a higher sensitivity and specific degrees,can be used as an important supplement of the conventional magnetic resonance imaging in the examination of prostate disease.There was a correlation between the ADC values and Gleason scores of prostate cancer.The ADC value of prostate cancer can provide a more accurate quantitative basis on classification,and can provide the choice of the treatment of prostate cancer with relatively reliable quantitative basis.The ADC values of magnetic resonance imaging can take noninvasively assessment of prostate cancer malignancy and prognosis.
