Objective: to study the TLR-4 and B7-H1 in the muscle layer of invasive bladder cancer(non muscle-invasive bladder cancer,NMIBC)BCG(Bacillus Calmette Guerin,BCG)the expression and clinical significance in patients with immune treatment failure;Bladder tumor of rats model was constructed,set B7-H1 block with BCG treatment group,the BCG group,B7-H1 block group and the blank control group,after the experiment through the comparison between groups of bladder tumor diameter and B7H1 expression to evaluate the therapeutic effect of each treatment,in order to confirm blocking B7-H1 / PD-1 pathway can enhance the immune effect of BCG treatment at high risk of bladder cancer.Methods: select 20 patients with high-risk NMIBC BCG postoperative perfusion treatment failure,using immunohistochemical staining method to detect patients with incipient and BCG treatment failure after bladder cancer tissues and B7 TLR-4-H1 protein expression;Build animal model of orthotopic bladder tumor of rats were randomly divided into B7-H1 block with BCG treatment group,the BCG group,B7-H1 block group and the blank control group,each group processed executed anatomy,snapping the rats to observe the rats bladder and bladder tumor,measuring each tumor diameter,testing each expression in bladder cancer cell surface B7-H1 molecule.Results: high risk NMIBC BCG immunotherapy after failure in patients with bladder cancer tissues and B7 TLR-4-H1 protein expression than before treatment were significantly increased,and the expression was significantly positive correlation,differences are obvious statistical significance;The bladder perfusion N-methyl phosphite nitrourea(MNU),the method of tumor rate as high as 95%.B7-H1 block with BCG bladder tumor diameter difference between treatment group and the blank control group,with statistical significance(p < 0.05),molecular expression level B7-H1 BCG infusion group was obviously higher than other groups.Conclusion: BCG immunotherapy in bladder cancer tissue in patients with high-risk NMIBC fail TLR-4 and B7 exist-H1 expression level,and failure could BCG immunization therapy for bladder cancer with B7-H1 expression mediated immune escape mechanism;BCG bladder cancer cells can be induced rats B7-H1 molecular expression increased significantly.Blocking B7-H1 / PD-1 pathway can enhance BCG treatment effect of bladder tumor.
