Systematic Review And Investigation On Current Situation Of Responsibility Holistic Nursing

Objective:1.To systematically review the domestic studies on responsibility holistic nursing,so as to provide suggestions for evidence-based strategic decision about how to set up a long-term mechanism of responsibility holistic nursing.2.To investigate nurses' cognition and behavior on responsibility holistic nursing in Chongqing.To explore the current situation and influencing factors of responsibility holistic nursing in secondary and tertiary hospitals in Chongqing.And analysing the problems in order to promote responsibility holistic nursing continuing to carry out.Methods:1.The databases of CMB,Wan Fang Data,VIP and CNKI were searched to identify the relevant studies.Literatures were screened with data extraction in accordance with the predefined inclusive and exclusive criteria.The quality of literatures was assessed with the standard of Cochrane Handbook 5.0 and Crombie Criteria,and the data were comprehensively analyzed with qualitative research methods.2.A total of 1674 nurses who came from secondary and tertiary hospitals in Chongqing were investigated by self-designed questionnaire,including demographic information,cognition and behavior on responsibility holistic nursing,the current situation and influencing factors of responsibility holistic nursing,and so on.Results:1.The total 33 cross-sectional studies were included,The results showed that: The cognition of responsibility holistic nursing is good;10 Literatures from the scope of business,organizational forms,strengthening the management and establish a system to expound how to implement responsibility holistic nursing;Since our country carry out responsibility holistic nursing,we have achieved some effects,but there are still some problems,discuss and solve problems by using the method of evidence-based is the future development direction.2.Questionnaire survey show:(1)More than 90% nurses had right cognition on responsibility holistic nursing;The cognitive score between secondary and tertiary hospitals were statistically different(P<0.01);There were statistically difference on cognitive score between those nurses who had different educational history(P<0.05).(2)100% nurses' department has carried out responsibility holistic nursing,and 57.3% nurses' department had carried out responsibility holistic nursing for more than 5 years;Responsibility holistic nursing behavior execution rate had achieved more than 70%;The behavioral score between secondary and tertiary hospitals were statistically different(P<0.01);There were statistically difference on behavioral score between those nurses who had different educational history(P<0.05);There were statistically difference on behavioral score among those nurses who came from different departments(P<0.01),ICU nurses' behavioral score is the highest,emergency nurses' behavioral score is the lowest.(3)More than 90% nurses believe implementing responsibility holistic nursing make patients get better care,improved patient satisfaction,and also enhanced ability of nurses;90.1% nurses had carried out transitional care,but the main ways were telephone intervention(96.9%)and lectures(67.8%);79.7% nurses took positive attitude toward continuing to carry out responsibility holistic nursing;More than 80% nurses believe that the main factors affecting responsibility holistic nursing are lacking of human resources and low pay of nurses.Conclusions:1.The current research about responsibility holistic nursing lack of randomized controlled trials,much as the cross-sectional survey research,but its research content is wide,analyzes the present status and effect of responsibility holistic nursing,summarizes the experience and problems,to promote our country to set up a long-term mechanism of responsibility holistic nursing has the certain enlightenment function.2.Chongqing secondary and tertiary hospitals has been widely carried responsibility holistic nursing,and have achieved remarkable results.Clinical nurses had a positive view on responsibility holistic nursing.But it still needs to ensure human resources and logistical support,reduce nurses' labor intensity,improve nurses' salaries,perfect the performance appraisal system,simplify nursing documents,perfect quality control system,in order to promote responsibility holistic nursing continuing to carry out.At the same time,it also should further promote the practice and research of transitional care,extending the responsibility holistic nursing to primary hospitals,communities and home care.
