| Objective: To investigate the relationship between free testosterone index(FAI)and insulin resistance(IR)in patients with polycystic ovarian syndrome(PCOS),and diagnostic performance evaluation of FAI in diagnosis of IR in PCOS.Methods: 139 patients as case group and 100 healthy adult women volunteers as control group were selected.All subjects conducted OGTT experiment and measured human basic indicators.blood glucose,blood lipids and other biochemical indicators were detected by enzymatic methods,hormones were determined by chemiluminescence,HOMA-IR was used to evaluate the level of IR.The level of serum FAI and HOMA-IR between PCOS group and control group were analysized by t-test,the correlation between FAI and IR were studied by Pearson correlation analysis and multiple linear stepwise regression,the improvement of FAI was evaluated by metformin intervention treatment,the best cut-off value of FAI in the diagnosis of IR was selected by ROC curve and diagnostic performance of FAI was evaluated,consistency of FAI associated with HOMA-IR in diagnosis of IR was assessed by McNemar chi-square test.Results: The levels of serum FAI and HOMA-IR in PCOS group were higher(P=0.00),After matching BMI,serum FAI and HOMA-IR levels of overweight group in PCOS patients were even higher(P=0.01);Pearson correlation analysis showed that serum FAI of PCOS patients were positively correlated with BMI(r= 0.42,P=0.42),HOMA-IR(r =0.46,P=0.00),FAT(r =0.25,P=0.25),HOMA-IR and BMI were the independent risk factors influencing the serum level of FAI in PCOS group by multiple linear stepwise regression analysis;After metformin treatment for six months,HOMA-IR of PCOS patients improved(P=0.00)following the improvement of FAI(P=0.00);For diagnostic performance of IR in PCOS patients,the sensitivity and specificity were 72.04 % and 65.22 %,respectively,the area under the ROC curve was 0.6891,95% CI was 0.5941~0.5941;The diagnostic consistency rate between FAI and HMOA-IR was 71.22%(99/139),there was no statistically significant difference between FAI and HOMA-IR by McNemar chi-square test(χ2=1.6,P=0.268).Conclusion: There is a positive correlation between FAI and IR in patients with PCOS,FAI has certain diagnostic value for IR. |
