Objective:To explore the characteristics of the flexible spectral imaging color enhancement and its application in the early diagnosis of laryngeal neoplasms.Methods:Collected from March 2015 to December 2016 63 cases suspected patients with laryngeal tumor,using EG-590 ZW electronic gastroscope,it has a white light endoscopy(white light endoscopy,WLE)and flexible spectral imaging color enhancemen of two kinds of observation mode,respectively for the laryngeal lesions were observed,photographed and preserved.Through the mucous membrane of the laryngeal lesion and the capillaries of the mucous membrane,the resolution of the boundary of the lesion and the lesion,the sharpness of the images was compared.By compared the correct diagnosis rate of two endoscopic modes;To compared the sensitivity and specificity of laryngeal and laryngeal cancer in two modes;In the endoscopy mode,the blood vessels in the larynx were divided.Results:1.The endoscope of FICE was significantly better than normal white light endoscopy((49)< 0.05)in the blood vessel morphology,appearance and lesion boundary.2.Eighty-five of lesions were found in 63 cases of laryngeal tumor lesions,including 28 benign lesions(including chronic inflammatory,polyps,and simple hyperplasia),mild atypical hyperplasia of 9,3 moderate atypical hyperplasia,3 severe atypical hyperplasia,6 orthotopic carcinoma,36 infiltrating cancer.The correct diagnosis rate for laryngeal cancer was 91 %.higher than68.2% of normal white endoscope,the difference was statistically significant(P< 0.05).The sensitivity of FICE endoscopy to precancerous lesions and laryngeal cancer was92.4%,higher than 74.6% of ordinary white endoscopy,which was statistically significant(P< 0.05).The specificity of FICE endoscopy was 91.8 percent specific to the precancerous lesion and laryngeal cancer,87.8 percent(P< 0.05)of ordinary white light endoscopy was not statistically significant.3.FICE classification and pathological diagnosis relationship.Patients with?type: 8 cases of polyp.Patients with?type:18 cases of chronic inflammation,The sensitivity of the diagnosis to benign lesions was92.8%.Patients with ? type:8 cases of mild atypical hyperplasia,3 cases of moderate atypical hyperplasia,and the sensitivity of the diagnosis of atypical hyperplasia was100%.Patients with ? type:1 case of mild atypical hyperplasia,2 cases of moderate atypical hyperplasia,6 cases of severe atypical hyperplasia and in situ carcinoma,1 case of invasive carcinoma,and the difference was statistically significant,Patients with ? type:infiltration cancer was 100%,and the sensitivity of its diagnosis was 97.2%(35/36).Conclusion:1.Compared with ordinary white light mode,the FICE model can clearly show the shape of the lesion profile,mucosal layer and submucosal microvascular,and enhance the identification of the lesion.2.FICE endoscopy for laryngeal cancer lesions and larynx disease diagnosis accuracy,sensitivity,than ordinary white light model has obvious advantages,To enhance the function of endoscopy in preoperative diagnosis and postoperative follow-up of laryngeal cancer and laryngeal cancer.3.The early lesions and small lesions of laryngeal tumors could be detected earlier by means of the FICE mode.
