| ObjectiveVertigo is a kind of motor hallucination,which is a common clinical symptom,It is a balanced system(visual,proprioception,the vestibular system)dysfunction and resulting in the absence of movement itself feel the movement of the head,Mostly caused by vestibular organ damage.The vestibular peripheral sensory system includes both the otolith and the semicircular canal system,Spontaneous nystagmus(spontaneous nystagmus SN)is one of the common signs of the vestibular function injury,the reaction is the balance of the static tension on both sides of the vestibular function.In this paper,the vestibular function of patients with acute unilateral peripheral vestibular injury was evaluated,analysis of the relationship between acute unilateral vestibular peripheral lesion and the intensity and direction of SN,to further explore the clinical value of SN in vestibular function and evaluation of patients with vestibular peripheral injury,and provide a reliable basis for the diagnosis and treatment of vertigo,vestibular function evaluation.MethodA total of 60 patients with Acute unilateral peripheral vestibular vertigo injury were enrolled in this study from November 2015 to July 2016 in the first central hospital of Tianjin.Otolaryngology,head and neck surgery,they were diagnosed as vestibular neuritis(Vestibular Neuritis VN);the last treatment time from the onset of vertigo within 15 days.SN(≥0.5 °/ s)and SN(<0.5 °/ s)were present in the patients,according to the general requirements for electronystagmogram vestibular function testing,including visual and vestibular ocular items,such as SN,head shaking nystagmus vestibular function test and caloric test.The intensity and direction of SN,the DP value and the UW value of the caloric test were used as the evaluation indexes(abnormal are called DP,UW),Qualitative and quantitative analysis of SN,DP,UW,the characteristics of SN、the relationship between SN and course of disease,DP and UW were analyzed According to the course combining with the course of disease.Result1、Among the 60 cases,there were 49 cases(81.7%)with SN,29 cases(18.3%)under the fixation condition,There were 44 cases(89.8%)of SN in caloric test DP,There are 42 cases of impaired response of semicircular canal(92.5%),unilateral weakness 35 cases(71.4%)with 4 cases having the direction of SN towards the weakness side and 31 cases pointing towards the opposite direction bilateral weakness 7 cases(14.3%)and normal 7 cases(14.3%).7 cases of BN in the course of disease was from 1d to 4d,above 5d,there were 5 cases and 2 cases respectively.2、SN intensity and duration of the disease were analyzed by regression analysis(F=10.292,P=0.02),and there was a linear correlation between the two groups.Further correlation analysis showed a negative correlation between the two.The strength of SN by light,and heavy group,the median disease duration is 6.5d,5d,3d,the difference was statistically significant,to compare the differences between groups,light group,medium group and severe group differences were statistically significant3、There were 49 patients with SN in 60 cases,44 patients(89.8%)with DP in caloric test,5 patients(10.2%)with normal DP value,53 patients(88.3%)with impaired response of semicircular canal,Semicircular canal response in normal 7 cases(11.7%).The intensity range of SN is 0.7 °/s to 20.4 °/s,and the mean value is 6.0°/s,and the DP intensity with SN ranged from 1% to 100%,with a median of 83%.SN intensity was positively correlated with DP;SN direction and DP direction were the same in 44 cases,and the reverse in the 5 cases.There was no correlation between SN intensity and UW value of caloric test.There are SN and caloric test UW 35 cases,no SN and UW in 11 cases.The median duration of UD was 5d and 5.5d,(40.9 ± 26.3)% and(29.9 ± 18.2)%,respectively.There was no significant difference between the two groups,the difference of UW value was statistically significant.ConclusionThe intensity of SN in patients with acute unilateral vestibular peripheral vertigo Majority was moderate and severe,Intensity with the course of the disease gradually weakened or change the direction,the degree of vestibular injury was higher than those without SN.SN has clinical guidance value in Peripheral peripheral vestibular injury and compensatory state assessment. |
