Analysis Of Maternal And Infant Outcomes After Successful Comprehensive Therapy To Maintain Pregnancy In Patients With Recurrent Spontaneous Abortion

ObjectiveEvaluate the security of the mother and infant after successful treatment to maintain pregnancy in recurrent abortion patients.Provide experimental data for clinical therapy.Methods1.Using the method of retrospective study,selected 105 cases of recurrent abortion patients who were admitted at Medical University Of Tianjin in Baodi Clinical School from June 2012 to June 2015,as the experimental group;105 normal pregnant women were selected randomly as control group.2.Therapy methods to maintain pregnancy in the experimental group: since the pregnancy was confirmed by the blood test human chorionic gonadotropin(beta-hCG),Patients who require for keeping the pregnancy were accepted in hospital,according to the relevant test.Progesterone,heparin or low molecular weight heparin,Aspirin Enteric-coated Tablets and glucocorticoid were used in therapy.3.Age,Gravidity and parity history,complications,hemorrhage and intr-apartum amniotic fluid were compared between the two groups.4.To observe pregnancy complications of the two groups,including gestational hypertension,gestational diabetes,placenta previa,postpartum hemorrhage,premature rupture of membranes,cholestasis,placental abruption;Cesarean section rate is also compared in two groups.5.Related laboratory indicators were observed in the two groups before the termination of pregnancy: platelet(PLT),platelet aggregation,antithrombin III(AT-III),liver function,renal function,blood coagulation.6.To observe fetal and neonatal conditions in two groups,including stillbirth,fetal malformation intrauterine growth restriction,premature and full-term low birth weight,full-term birth weight,Apgar score,macrosomia,neonatal malformation phenotype,sexual development abnormality,neonatal sex ratio and new Birth defects;neonates are followed-up to 6 months after birth.7.The data were analyzed by SPSS 17 software.using Chi square test and t test,if the P<0.05 it has statistically significant.Results1.The maternal age of the experimental group was 29.03 ± 5.16 years,the gravidity was 3.81±1.05;The maternal age of the control group was 26.62±4.28 years,the gravidity was 1.60 ±0.82,the difference was statistically significant(P<0.05).There were no significant differences between the two groups in the parity history and pregnancy complications(P > 0.05).The intrapartum hemorrhage and amniotic fluid comparison score were no statistically significant differences in the two groups.(P > 0.05).2.In the experimental group,the incidence of gestational hypertension was 1.9%,the rate of premature rupture of fetal membranes was about 5.7%,in the control group,the incidence of gestational hypertension was 8.6%,the incidence of premature rupture of fetal membranes was about 18.1%,the difference was statistically significant(P < 0.05).The incidence of gestational diabetes,placenta previa and postpartum hemorrhage rate were 2.9%,1.0%,1.9% respectively in the experimental group.The incidence of gestational diabetes,placenta previa and postpartum hemorrhage rate were 2.9%,1.9%,1% respectively in the control group,,there was no statistically significant difference(P > 0.05);There were no cholestasis and placental abruption happened in the two groups.3.The index of D-Dimer,platelet aggregation rate,AT-III,PLT,PT,APTT and Fib before delivery in two groups were no statistically significant difference(P > 0.05).4.There was no significant difference between the two groups in ALT and AST before the delivery(P > 0.05);there was no significant difference in BUN and Scr(P > 0.05).5.The cesarean section rate of the experimental group was 71.57%,and the rate of cesarean section in the control group was 48.54%,the difference was statistically significant(P < 0.05).6.The number of fetus was both 105 in the two groups with singleton pregnancy.There were one case of fetal growth restriction,2 cases of fetal death and 1 cases of fetal malformation in the experimental group.2 cases of fetal malformation were found in the control group,no fetal growth restriction and fetal death were found in the control group.There is no significant differences between the two groups(P > 0.05).7.There were 7 premature infants in the experimental group and 6 premature infants in the control group wtih no significant difference(P > 0.05).8.There were 95 cases of full-term live births and 1 case of low birth weight infant in the experimental group.There were 97 live births with no low birth weight infants in control group.There was no significant difference in the neonatal birth weight(P > 0.05).9.There were 102 neonates in the experimental group and 103 cases in the control group.Birth score,sex ratio,macrosomia,neonatal birth defects [including multiple(and)finger(toe)and adactylism(toe),ear,atresia,hypospadias,congenital heart disease,congenital cleft lip and palate,no anal] in the two groups have no significant differences(P > 0.05);the experimental group and the control group had There were no cases of stillbirth,neonatal asphyxia,neonatal mortality,neonatal anemia,electrolyte disorder and sexual development abnormality in the two groups;Special face characterized by abnormal chromosomes were not found in the two groups.All neonates were followed up to 12 weeks,there was no significant difference in the incidence of jaundice between the two groups(P > 0.05),up to 6 months,there was no infant death occurred.There was no significant difference(P > 0.05).Conclusion1.Heparin or low molecular weight heparin,aspirin Enteric-coated Tablets can effectively improve the prethrombotic state in Patients with recurrent spontaneous abortion in early pregnancy,At the same time it does not increase the risk of Intrapartum hemorrhage and postpartum hemorrhage.2.Natural progesterone therapy can improve the decrease of progesterone due to luteal insufficiency,and can effectively improve the endometrial receptivity at the same time with no increase of the risk of sexual development abnormality.3.Prednisone in the treatment of RSA in early pregnancy with the appropriate dose can effectively suppress the adverse immune response and inhibit rejection of maternal immunity can improve the success rate of pregnancy.4.Traditional Chinese medicine treatment is safe and effective in treating RSA patients with no increase in fetal malformation risk.5.Comprehensive treatment of patients with recurrent spontaneous abortion is safe and effective,without adverse effects and can promote in clinical application.
