ObjectiveRetrospective analysis of clinical data of inpatients suffering from senile pneumonia in an A level hospital of Qiqihar,looking for the cases characteristics and clinical manifestation of senile pneumonia,exploring the factors affecting the death of patients suffering from senile pneumonia.These will provide scientific basis for the prevention and treatment of senile pneumonia in Qiqihar.MethodAdopting the retrospective analysis method of medical cases,collecting and sorting out the medical cases data of inpatients with senile pneumonia,describing the general demographic characteristics,clinical features and laboratory examination medical cases characteristics of patients suffering from senile pneumonia,analyzing the influence factors which causing the death of patients suffering from senile pneumonia.Using SPSS20.0 statistical software for statistics analysis,describing the rate of technical information and constituent ratio,different groups were compared by chi-square test or Fisher exact probability method or rank test.Binary Logistic regression analysis was adopted to find out the influencing factors causing the death of patients suffering from senile pneumonia.Result1.In this survey,the elderly 363,accounting for 58.5%;257 young people,accounting for 41.5%.Suggested to strengthen the screening of young and middle-aged colorectal cancer;male colorectal cancer patients occupy a high proportion of 59.4%,it is recommended to strengthen the screening of male2.Among the collected 347 medical cases,there are 281 male patients cases(accounting for 80.98%),66 female patients cases(accounting for 19.02%).Seasonal incidence,in Spring,there are 87 cases(accounting for 25.07%),in Summer,there are 83 cases(accounting for 23.92%),in Autumn,there are 93 cases(26.80%),in Winter,there are 84 cases(accounting for 24.21%);For smoker patients,there are 207 cases(accounting for 59.65%),for drinker patients,there are 184 cases(accounting for 53.03%);in activity of Daily Living(Barthel index),there are 217 mild dependency cases(accounting for 62.54%),91 severe dependence cases(accounting for 26.22%),39 moderate dependency cases(11.24%).Three typical symptoms in clinic are fatigue,anorexia,lethargy/somnolence,respectively accounting for 97.41%,96.54%,and 95.10%;72 cases with body temperature>37.2?(accounting for 20.75%);255 cases lung rale(accounting for 73.49%);three high complications incidence of combined disease were heart disease,hypertension,cerebrovascular disease,respectively,60.52%,53.31%,46.4%.3.Increased blood glucose 151 cases(accounting for 43.52%),reduced potassium ions 53cases(accounting for 15.27%),reduced Sodium ions 52 cases(accounting for 14.99%),reduced chloride ions 37 cases(accounting for 10.66%),reduced calcium ions cases(accounting for 10.66%).Patients with white protein reduced 230 cases(accounting for 89.34%),reduced prealbumin 267 cases(accounting for 76.95%),reduced hemoglobin 114 cases(accounting for 32.85%);increased creatinine 95 cases(accounting for 27.38%),increased urea nitrogen 165 cases(accounting for 47.55%).4.Patients with senile pneumonia elevated WBC 114 cases(accounting for 32.85%),increased neutrophil percentage 184 cases(53.03%),reduced lymphocyte percentage 214 cases(accounting for 61.67%),increased PCT 100 cases(accounting for28.82%),increased CRP 236 cases(accounting for 68.01%)5.Patients with the positive pathogenic bacteria 133 cases(accounting for38.33%);the positive rate of diagnostic imaging tests is 90.49%,of which two lungs are involved accounts for 74.06%.6.Patients with senile pneumonia died 37 cases,accounting for 10.66%,single factor analysis shows that Barthel index classification,dyspnea,nausea,hypotension,lung rale,hyponatremia,hypocalcaemia,elevated aspertate aminotransferase,reduced albumin,anemia,increased urea nitrogen,elevated white blood cells,increased neutrophil percentage,the rate of myelogenous leukemia and lymphocytic leukemia increased,increased PCT,the culture-positive of sputum,antibiotics usage,the rate of myelogenous leukemia and lymphocytic leukemia---all these are the factors of leading to the deaths of patients with pneumonia(P < 0.05).7.The multivariate logistic regression analysis results show that Barthel index classification heavy dependence(OR=5.763,95%CI=2.325 ~14.280),hypotension(OR=4.649,95%CI=1.375 ~15.723),hypocalcaemia(OR=6.668,95%CI=2.512 ~17.695),increased urea nitrogen(OR=3.980,95%CI= 1.572~10.075)are the independent risks affecting the deaths of the patients with senile pneumonia.Conclusion1.Patients with senile pneumonia are mainly elderly,and there was no apparent seasonality in senile pneumonia patients.2.Patients with senile pneumonia have no classical clinical symptoms with many complications.3.Patients with senile pneumonia tend to respond slowly with unobvious high temperature and increased white blood cells.4.Patients with senile pneumonia are prone to low potassium,low sodium,low calcium and other electrolyte disorders.5.Patients with senile pneumonia are more common with bacterial pneumonia.6.The mortality rate of elderly pneumonia patients is high,and the Barthel index classification heavy dependence,hypotension,hypocalcaemia,increased urea nitrogen are the independent risks affecting the deaths of the patients with senile pneumonia.
