| BackgroundShorten the average hospital stay is an important research topic of hospital management,and long-term hospitalization is one reason for long hospital stay,not only does it increase the economic burden of patients,but also waste a lot of valuable medical resources,there is no study on the distribution characteristics of long-term hospitalization in hospitals of Suzhou.ObjectiveTo analyze the relationship between the epidemiological characteristics(including gender,age,department,disease,complications,medical expenses and treatment results)of long term inpatients and the length of hospital stay,and to provide evidence for institute measures to shorten the average hospital stay.Subjects and methodsCollecting information of inpatients from the Hospital Information System and outpatient reservation management system of a tertiary hospital in Suzhou during 2013 to2015.6454 patients who stayed in hospital for more than 30 days were selected as the research subjects.Organized the data with Excel2007 and used SPSS19 for statistical analysis,the significant level was 0.05.Result1.There were 252,600 inpatients in 2013-2015,6454 of them were hospitalized for more than 30 days,accounting for 2.55%of all inpatients.Most of them were male,middle aged and elderly people;the median medical expenses for the three years were 94,100yuan,117,200 yuan and 119,700 yuan;and the median length of stay was within 40 days.2.The hospital stay for 30-40 days accounting for the largest proportion in long term inpatients in 2013-2015,the constituent ratio were 52.8%,50.6%and 52.9%,respectively;and the constituent ratio of more than 60 days hospitalization was 11.1%,13.1%and10.9%,respectively(?~2=10.312,P=0.006).The proportion of<40 days in both sexes was the largest in the three years,and there was no statistical significance in the length of hospital stay in different genders(P>0.05).3.The number of patients with long-term hospitalization in department of hematology,department of infectious diseases,department of respiration,department of cerebral surgery and radiotherapy department were in the top five;the constituent ratio of hospital stay between 40 to 50 days was the largest in radiotherapy department(36.2%),the constituent ratio of hospital stay≥60 days was the largest in department of infectious diseases(17.2%)(?~2=137.753,P<0.0001).4.The number of patients with long-term hospitalization in acute lymphoblastic leukemia,preoperative malignant tumor chemotherapy,pulmonary infection,acute myeloid leukemia M2 and myelodysplastic syndrome were in the top five;the constituent ratio of hospital stay<40 days was the largest in all diseases mentioned above,the constituent ratio of hospital stay≥60 days was the largest in a patient with pulmonary infection(17.3%)(?~2=19.967,P=0.001).5.The constituent ratios of the patients without complications in<40,40-50,50-60and≥60 hospital day were 56.5%,23.2%,11%and 9.3%,respectively.The corresponding constituent ratios of the patients with complications were 52.3%,24.4%,11.1%and 12.1%,respectively.There was s statistical significance in the length of hospital stay(P=0.008).6.The longer the hospital stays,the more medical expenses were spent(r_s=0.299,P<0.0001);the average daily medical expenses between 1493-2650 yuan in 50-60 and≥60hospital day had the largest proportion,accounting for 12.9%and 12.4%,respectively;average daily medical expenses≥4852 per day accounted for the minimum proportion in≥60 hospital day(?~2=12.469,P<0.006).7.The constituent ratio of cured patients was the largest(61.6%)in<40 hospital day patients,death and unhealed patients had bigger constituent ratio with≥60 hospital day,the proportion was 24.1%and 23.3%,respectively(?~2=24.551,P<0.0001).Conclusion1.Long term inpatients were mainly male,middle aged and elderly people,the total cost of medical treatment is about 100,000 yuan,and the median length of stay was within40 days.2.There were more long-term inpatients in department of hematology,department of infectious diseases,department of respiration,department of cerebral surgery and radiotherapy department,moreover,the inpatients with over length hospitalization in department of infectious diseases were more than those in other departments.3.There were more acute lymphoblastic leukemia,preoperative malignant tumor chemotherapy,pulmonary infection,acute myeloid leukemia M2 and myelodysplastic syndrome in long-term inpatients,longer time hospitalized patients were more likely to suffer from pulmonary infection than other diseases.4.The patients with complications had longer hospital stay than those without complications;the longer the hospital stays,the more medical expenses were spent,instead,it had less average daily medical expenses;compared to death and unhealed patients,patients had shorter hospitalization time. |
