Objective: With the deepening of the aging of the population in China,the demands of the elderly for health have also changed dramatically.Starting from the real world of the elderly inpatients,this study conducted mining and analysis on the first page data of more than 350000 person-times of inpatient medical records.The first is to understand the population characteristics of the elderly inpatients,the second is to conduct in-depth analysis on the main systemic diseases and single diseases from different dimensions in order to understand the disease composition and distribution characteristics,and the third is to analyze the composition and main influencing factors of the elderly inpatient expenses,It provides the basis for the adjustment of hospital medical resources and the optimization of the elderly medical service strategy.Methods: Extract the information on the first page of the medical records of elderly inpatients(≥60 years old)from January 1,2016 to December 31,2021 in a tertiary hospital in Liaoning Province and conduct data cleaning,establish a database of elderly inpatients,describe the population characteristics of elderly inpatients and disease composition and distribution characteristics using descriptive analysis methods,extract the co-occurrence relationship of disease diagnosis and generate the co-occurrence network diagram of disease diagnosis,and mine the co-disease model of elderly inpatients,The influencing factors of hospitalization expenses of elderly patients were studied by single factor analysis and multiple factor analysis.Results: A total of 351623 inpatient medical records of elderly patients were included in this study.1.Population characteristics: In addition to the impact of the COVID-19 in2020,the number of inpatients is increasing year by year.The gender ratio of male and female inpatients is 1.12:1.Patients aged 60-69 account for the highest proportion.More than 95% of patients come from Liaoning Province.The average length of stay of patients is 8.99 days and is decreasing year by year.The older the patients are,the longer the average length of stay is.2.Disease composition and distribution characteristics: The proportion of patients suffering from two or more diseases is as high as 78.92%.Hypertension is the most common comorbidity,and hypertension diabetes is the most common comorbidity mode.Most of the comorbidity relationships of diseases with high comorbidity frequency include chronic non infectious diseases.In the ranking of systemic diseases,tumors,circulatory system diseases and digestive system diseases have always been in the forefront,of which lung cancer is the most common single disease,The proportion of male patients with most systemic diseases is higher than that of female patients.With the growth of age,circulatory diseases replace tumors as the main reason for hospitalization of elderly patients.The proportion of patients hospitalized with respiratory diseases in winter and spring is higher than that in summer and autumn.3.Composition of hospitalization expenses and influencing factors: the average hospitalization expenses of the hospital in 6 years have increased overall,with the largest proportion of consumables,the highest total hospitalization expenses of patients hospitalized for tumor,and the highest average hospitalization expenses of patients hospitalized for disease and death due to external causes.The results of single factor analysis and multiple factor analysis show gender,age,hospitalization days,number of operations and operations,admission route,main diagnosis of admission condition Seasons are all the factors that affect the hospitalization expenses of the elderly patients in the hospital,and the hospitalization days and the number of operations and operations are the important factors that affect the hospitalization expenses.Conclusion: The number of elderly inpatients in this hospital is increasing year by year,and it is recommended to focus on the multimorbidity and common diseases of the elderly in summer and autumn,strengthen health education for the male elderly,and meet the demand of health services for the elderly aged 60-69 years;Elderly hospitalized patients are often in a state of multimorbidity,suggesting that governments and medical institutions should strengthen the tilt of medical resources for comorbidities of hypertension diabetes,strengthen popular science education education about chronic non-communicable diseases such as hypertension,improve prevention and control of malignant tumors among older adults aged 60-79 years,focus on the impact of musculoskeletal system and connective tissue disorders on the health of female older adults,Alert to the damage of circulatory diseases to the healthy lifespan of older adults;In response to the excessive increase in the cost of secondary average hospitalization,it is recommended to reduce the ratio of consumables costs and focus on the burden of disease caused by chronic non-communicable diseases,medical institutes should rationally configure medical and health resources according to the composition and distribution characteristics of diseases in the elderly,optimize medical treatment strategies,control the additional cost of surgery and operation and reduce the length of hospital stay within a reasonable range,and control the hospitalization cost is not reasonable to increase.
