Part one:PET-like Imaging,Relevant Research of PET-like Stages and Clinical Stages in Multiple MyelomaObjectives:To analyze the PET-like images of whole-body magnetic resonance of multiple myeloma.To explore the relationships between PET-like stages and clinical stages.Methods:28 multiple myeloma patients were enrolled.Pathological or clinical proofs of multiple myeloma were obtained in all cases.Fisher's exact test was used to analyze MRI stages and clinical stages.Results:28 cases were classified as normal type(2 cases),mildly infiltrating type(6 cases),moderately infiltrating type(12 cases),severely infiltrating type(8 cases).Clinical stages of 28 cases were comprised of 7 cases of stage ?,10 cases of stage ?,11 cases of stage ?.A P value under 0.05 was considered to be significant for the comparative analysis.And there was no statistically significant difference between PET-like image stages and clinical stages.Conclusions:The PET-like images of whole-body MRI had good image effect and evaluation value for multiple myeloma.Part two:Comparative Research of Whole-body T2WI and PET-like Imaging in Multiple MyelomaObjectives:To further research the PET-like images of whole-body magnetic resonance of multiple myeloma.And in contrast with T2WI in order to discuss their sensitivity.Methods:45 multiple myeloma patients were enrolled.Pathological or clinical proofs of multiple myeloma were obtained in all cases.Conduct contrast research of manifestations and positive rates.Results:MM can be divided into normal?focal?diffuse?focal and diffuse?"salt-and-pepper" patterns in T2WI.45 cases were classified as normal type?mildly infiltrating type?moderately infiltrating type?severely infiltrating type in PET-like imaging.The positive rate of PET-like imaging was higher than T2WI except skull and neck lymph nodes.Conclusions:The PET-like sensitivity of MM was higher than T2WI except skull and neck lymph nodes..Part three:Comparative Study of PET-like Imaging with Low-dose CT Imaging in Multiple MyelomaObjectives:Research the features of PET-like imagings and low-dose CT imagings in multiple myeloma;Discuss sensitivity of PET-like imagings and low-dose CT imagings in multiple myeloma.Methods:6 multiple myeloma patients were enrolled.Pathological or clinical proofs of multiple myeloma were obtained in all cases.Whoe-body PET-like examinations were carried out at Siemens 3.0T MRI and whoe-body low-dose CT examinations were carried out at Siemens Dual Source CT in our hospital.Comparative study of PET-like imagings with Low-dose CT imagings in multiple myeloma.Results:MM lesions were dotted and patchy low signals in PET-like examinations;multiple bone destruction with no sclerotic rims or no periosteal reaction in low-dose CT examinations.The PET-like sensitivity of MM was higher than Low-dose CT Imaging except the skull.Conclusions:The sensitivity of PET-like imagings were higher than low-dose CT imagings in multiple myeloma except the skull.Part four:Applied Research of Whole-body Multiple b Values PET-like Imaging of Multiple Myeloma in Siemens 3.0T ScannerObjectives:To investigate the characteristics of whole-body multiple b values PET-like imaging of multiple myeloma in Siemens 3.0T scanner;Compare the differences of multiple b values PET-like imaging in MM;Discuss the optimization of b value.Methods:19 multiple myeloma patients were enrolled.Pathological or clinical proofs of multiple myeloma were obtained in all cases.The b values of 50s/mm2?400s/mm2?700s/mm2 were selected respectively.Analysize the differences of multiple b values PET-like imaging in MM and carry out the control study.Results:There were differences of singal between MM lesions and intervertebral discs.MM lesions were slightly lower singal at b value of 700s/mm2.The signal to noise ratio(SNR)and contrast to noise ratio(CNR)reduced and the sensitivity for MM lesions detection increased.A P value under 0.05 was considered to be significant for the singal differences between MM lesions and intervertebral discs with multiple b values by one-way ANOVA,SNR and CNR were also like this.Conclusions:MM PET-like scanning with Siemens 3.0 T can be used to distinguish MM lesions and intervertebral discs by multiple b values(50,400 and 700s/mm2).The diagnostic value was the highest when b value was 700s/mm2 and the b value of 700s/mm2 can improve the sensitivity of lesion detection.Part five:Preliminary Research of Whole-body MRI PET-like Imaging in Durie-Salmon Staging SystemObjectives:To analysize the correlations between MRI PET-like imaging of multiple myeloma and bone marrow infiltration;Discuss the value of whole-body MRI PET-like imaging in Durie-Salmon staging system.Methods:68 multiple myeloma patients were enrolled.Pathological or clinical proofs of multiple myeloma were obtained in all cases.The PET-like imagings of MM were divided into four types of normal?mild infiltrating?moderate infiltrating and strong infiltrating according to the number of lesions and the value of ADC.Results:47 patients were in the higher stage of Durie-Salmon staging system;A P value under 0.05 was considered to be significant for the difference between Durie-Salmon staging system and Durie-Salmon PLUS PET-like staging system by McNemar-Bowker's test.Conclusions:The value of PET-like imaging to Durie-Salmon staging system was great.PET-like imaging may be included in Durie-Salmon staging system by my study.
