Objective:To evaluate the different efficacy between Microfracture and Collagen type ?matrix treating the cartilage defeats,to provide evidence for clinical treatment.Methods:9 patients were treated with Collagen type ? matrix(COL)and 8 patients were treated with Microfracture(MF).The clinical outcome was assessed preoperatively and at 3,6 and 12 months after surgery using the International Knee Documentation Committee(IKDC)score and Lysholm knee score scale.Magnetic Resonance Imaging,Magnetic Observation of Cartilage Repair Tissue(MOCART)score and T2 mapping were assessed at the same time.Results:17 patients were included in this study,with a mean age(years)of 33.4±10.1,a mean BMI(kg/m2)of 23.7±3.1 and a mean cartilage defeats area(cm2)of 1.3±0.5.Postoperative clinical scores of all patients were gradually rising and significantly higher than before.The mean IKDC values of the two groups at 12 months postoperatively were 81.4±9.6 and 90.2±4.3,significantly improving from the preoperative values(p<0.05),and the MF group improved more obviously than the COL group(p<0.05).The mean Lysholm values of the two groups at 12 months postoperatively were 91.4±6.9 and 91±3.2,significantly improving from the preoperative values(p<0.05),and there were no obvious differences between each other.Significant improvement of the MR images was observed after 6 months and remained to 12 months postoperatively in all patients.The mean MOCART values of the two groups at 12 months postoperatively were 71.0±11.9 and 79.2±5.8,significantly improving from the preoperative values(p<0.05),and the MF group improved more obviously than the COL group(p<0.05).The T2 values of the two groups at 12 months postoperatively were 39.2±6.7 and 30.3±4.0,significantly declined than 3 and 6 months,and no obvious difference were observed between repair area and normal cartilage(p>0.05).Conclusion:Collagen type ? matrix(COL)showed excellent effect in the treatment of cartilage defects.cartilage defeats were repaired effectively at 12 months after operations,but no advantages were showed comparing with the traditional Microfracture(MF).T2 mapping can monitor the cartilage repairing noninvasively,dynamically and quantitatively,provides a more objective evaluation of cartilage repair process postoperative.
