Objective: To investigate the application value of the spectral CT imaging in the differential diagnosis of xanthogranulomatous cholecystitis and gallbladder carcinoma.To investigate the application value of the spectral CT imaging in the differential diagnosis of chronic cholecystitis and gallbladder carcinoma.Methods: In this retrospective study,a total of 26 patients including 5 cases of xanthogranulomatous cholecystitis and 21 cases of gallbladder carcinoma in the biliary tract surgery department ?of Eastern Hepatobiliary Surgery Hospital affiliated to the Second Military Medical University underwent abdominal tri-phase enhanced spectral CT imaging examinations before operations and all pathologically confirmed from January 2015 to November 2016.The spectral parameters of the lesions were analyzed.Two groups of parameters were analyzed with two samples t test analysis.The ROC curve analyzed to get the sensitivity and specificity of the corresponding.In this retrospective study,a total of 87 patients including 57 cases of chronic cholecystitis and 30 cases of gallbladder carcinoma in Eastern Hepatobiliary Surgery Hospital affiliated to the Second Military Medical University underwent abdominal tri-phase enhanced spectral CT imaging examinations before operations and all pathologically confirmed from September 2015 to December 2016.The spectral parameters of the lesions were analyzed.Two groups of parameters were analyzed with two samples t test analysis.The ROC curve analyzed to get the sensitivity and specificity of the corresponding.Results:At the arterial phase and portal venous phase,?Hu,iodine(water)concentration,NIC and effective atomicnumber were with statistically significant(P<0.05),with no statistically significant(P>0.05)at equilibrium phase.ROC curve analysis showed that ?Hu,iodine(water)concentration at the arterial phase have high accuracy in the differential xanthogranulomatous cholecystitis and gallbladder carcinoma,with sensitivity and specificity for(85.7 %,80.0 %),(85.7 %,80.0%).At the arterial phase,portal venous phase and equilibrium phase,?Hu,iodine(water)concentration,NIC and effective atomicnumber were with statistically significant(P<0.05).The EAD and EPD of chronic cholecystitis were significantly higher than those of gallbladder carcinoma,with all statistically significant(P<0.05).The PAD of chronic cholecystitis and gallbladder carcinoma had no statistically significant(P>0.05).ROC curve analysis showed that ?Hu,iodine(water)concentration at the arterial phase have high accuracy in the differential chronic cholecystitis and gallbladder carcinoma,with sensitivity and specificity for(80.0%,82.5%),(80.0%,82.5%).Conclusion: Spectral CT imaging has high diagnostic value for differential diagnosis of xanthogranulomatous cholecystitis and gallbladder carcinoma.Spectral CT imaging has high diagnostic value for differential diagnosis of chronic cholecystitis and gallbladder carcinoma.
