Background: It is a threat to people’s lives that acute coronary events happen.Research shows that the majority of the acute coronary event is caused by unstable plaque rupture,which possesses the following characteristics: thin fibrous cap,big lipid nucleus,inflammatory infiltrates and bleeding internal plaque,positive remodeling.Discovering potential factors which influence the stability of the plaques is becoming a hot topic of current research.Current detection methods of unstable plaque mainly include imaging examination and blood marker detection,and imaging examination mainly include:(1)Intravascular ultrasound imaging: which can accurately display the location/shape/size/texture/surface rupture and ulcers of the plaques,but the resolution is relatively low,the fibrous cap and inner-middle film border distinguish is insufficient;(2)Optical coherence tomography: which has the highest space resolution,about 10 um resolution,is a reliable means of identifing unstable plaque,can accurately measure the thickness of the fibrous cap,discerning patchs lipid composition,but the price is relative expensive;(3)Multi-slice computed tomography:which with high spatial resolution,according to location of the plaques wall,can assess the overall state of plaque,from volume,low CT attenuation,napkin ring sign,positive remodeling,punctate calcification to endothelial shear characteristics suggest the presence of high-risk plaque,but the microscopic structure of evaluation is limited.Above imaging methods because their low degree of differentiation,heavy economic burden and traumatic,limiting theirs clinical large-scale popularization.Objective: To explore serum levels of thioredoxin and nuclear transcription factors and the relationship between the coronary plaque instability,and whether it has clinical value to the diagnosis of acute coronary syndrome.Methods: Collecting the patients with complained of chest pain from January 2016 to February 2017 being hospitalized in loudi center hospital,parallel,with coronary angiography patients 170 cases.Through history inquiry and physical examination,improve heart colour to exceed,electrocardiogram,chest X-ray,such as auxiliary examination,statistics of coronary lesion blood vessel counts,according to the result of the CAG and clinical manifestation,laboratory examination,electrocardiogram dynamic change were divided into CHD group 132 cases and control group 38 cases,CHD group were then divided into ACS group 68 cases and SAP 64 cases.Application of Siemens S2000 color doppler ultrasonic diagnostic instrument for all entrants carotid ultrasound examination,observing and recording whether to have carotid artery plaque formation.After admission to the patient’s general data are collected,with the method of ELISA detecting Trx and NF-κB.SPSS19.0 is used to analyse the data,p < 0.05 for the difference was statistically significant.Results: 1.The serum Trx /NF-κB level of CHD group is higher than the control group[(388.82±36.74)vs(302.94±36.91)ng/l,p <0.05]/[(1538.87±80.71)vs(1231.99±109.71)ng/l,p <0.05].2.The serum Trx /NF-κB level of ACS group is higher than in SAP group[(434.80±26.88)vs(388.82±36.74)ng/l,p <0.05]/[(1632.55±105.75)vs(1538.87±80.71)ng/l,p<0.05].3.The serum Trx /NF-κB levels of double branch lesion group is higher than single lesion group[(423.26±24.20)vs(348.98±26.57)ng/l,p<0.05] /[(1629.05±114.08)vs(1350.47±113.50)ng/l,p<0.05];the serum Trx/NF-κB levels of multivessel lesions is higher than double lesion group[(473.10±29.83)vs(423.26±24.20)ng/l,p<0.05]/[(1844.28±127.44)vs(1629.05±114.08)ng/l,p<0.05].4.The serum Trx/NF-κB levels on the early diagnosis of ACS having a certain value,respectively in the area under the ROC curve were 0.815 and 0.825,both joint in the area under the ROC curve is 0.853.Conclusions: 1.The serum Trx/NF-κB levels is maybe involved in the occurrence and development of CHD.2.The expression of serum Trx/NF-κB increase diagnosis of ACS maybe has a certain clinical significance.
