Objective :Through the analysis and comparison of the difference of mRNA expression in the pressure injury,burn acute wound and normal tissues,such as adhesion molecule(VCAM-1,ICAM-1)?inflammatory factors(IL-1??IL-6?TNF-?)?growth factor(VEGF,TGF-?)and other cytokines,to explore the possible internal mechanism and the chronic refractory healing of press ulcer.It can provide clinical theory for the treatment of pressure ulcer and other chronic refractory wounds Methods:Collect the tissues of clinical patients who had been diagnosed,including stage 3or 4 pressure injury tissues,burn acute wound tissues and wrapping tissues,divided into experimental group(pressure group)13 cases,control group(burn group)9cases,control group(normal)10 cases.By the RT-PCR technique analysis to compare the genes mRNA expression of related cytokines in each group,including VCAM,ICAM,ILs,TNF,VEGF,TGF.Results:1?The expression of VCAM-1 mRNA in pressure injuryr group and burn group were 18.74±6.084 and 333.3±41.33,there were significant difference(P<0.001),the two groups were higher than normal control group,the difference was statistically significant(P<0.05),and from high to low were burn group,pressure ulcer group,In the normal group,the expression of ICAM-1 mRNA in pressure injury was lower than that in the other two groups,P was less than 0.05,and from high to low were the burn group,the normal group and the pressure injury group;2?By RT-PCR technique to study the expression of inflammatory factors ILs and TNF-? in different wound tissues,The results showed that the expression of IL-1? and IL-6mRNA in the pressure injury group was significantly higher than that in the normal group(P<0.01),while the expression of TNF-? mRNA in the pressure injury group was significantly lower than that of the other two groups,and there were significant differences(P<0.001);3?The expression of growth factor VEGF and TGF-? mRNA in the pressure injury tissues and burn group was 0.03461±0.009849 and 0.3725±0.1785,and there were significant differences(P<0.01).The pressure injury group was significantly decreased compared with the two control groups,and the difference was statistically significant(P<0.05).The expression of TGF-? m RNA in the pressure injury tissues and burn group was 0.4895±0.1965 and 39.04±8.626,the difference was significant(P<0.001).Conclusion:The high expression of adhesion molecule VCAM-1 and the lack of ICAM-1and the high expression of inflammatory cytokines and the decrease of growth factor are not beneficial to the wound healing,which may be the intrinsic molecular mechanism of the refractory wound healing of pressure injury.
