Clinical Analysis Of 24 Cases Of Rhabdomyolysis

Objective:To explore the pathogenesis,clinical features,treatment and prognosis of rhabdomyolysis(RM)and to improve the understanding of rhabdomyolysis.Methods :Retrospective analysis of 24 cases of rhabdomyolysis diagnosed between November 2013 and November 2017 in the emergency department of Nanchang University Hospital,with general data(including age,sex,occupation,past history),etiology,clinical signs,laboratory examination indicators for admission(including serum enzymes,blood creatinine,urea nitrogen,electrolyte,blood routine,urine routine,electrocardiogram,etc.),treatment and prognosis.Results:In 24 patients with rhabdomyolysis,the data were not specific.4 cases of drug poisoning(16.7%),4 cases of infection(16.7%),4 cases of multiple injuries(16.7%),2cases of grand mal seizures(8.3%),2 cases of vigorous exercise(8.3%),3cases of crayfish poisoning(12.5%),1 case of thermonucleosis(4.2%),1 case of alcoholism 4.2%),2 cases of diabetic ketosis(8.3 %),There were 10 cases of muscle weakness(41.7%),10 cases of muscle redness(41.7%),8 cases of muscle pain(33.3%),7 cases of tan urine(29.2 %).Laboratory examination indicators: creatine kinase(38402.63±41191.97 U/L),CK-MB(680±1002U/L),SCr(264±316umol/L),UN(16.46±7.3 molol),AST(1021±2439U/L),ALT 678.3±435.5U/L),LDH(5163,163±9051 U/L);Blood CK value & GT;17 out of 20 patients with 5000 U/L complicated acute renal failure(85 % incidence);Urinary routine indicated that 2 cases were positive(8.3 %)and 7 cases were positive(29.2 %).Of the 24 patients,23 were improved and 1 died.Average day of hospitalization of 24 patients.Conclusions:The age,sex and history of rhabdomyolysis were not specific.Most of the causes are poisoning,trauma,clinical manifestations,the severity of the disease varied,the typical manifestations are muscle weakness,muscle redness,muscle pain,tan urine,often concurrent acute renal failure;CK value & GT;At 5000 U / L,the incidence of acute renal failure caused by RM was higher.The curative effect of combined medical treatment and blood purification is good.
