Objective To observe the clinical condition of the patients with type 2 diabetes mellitus in the clinic,to analyze the related risk factors of diabetic retinopathy in patients with type 2 diabetes mellitus.Method Randomly selected in our hospital department of endocrinology in July 2014-2016 July 2 patients with type diabetes were treated with a total of 529,including 176 cases of diabetic retinopathy(DR)patients,and 353 cases of Non-Diabetic retinopathy(NDR)Patients.Questionnaire investigation and laboratory test of related indicators were administered,The main information collected include name,gender,age,occupation,basic disease,personal history,history of allergies,family history,diabetes history,medication history,diet and exercise.At the same time,make the physical examination of patients,including body height,body weight,blood pressure,respiration,heart rate,body temperature,etc.Laboratory tests mainly contain blood related indicators : Serum total cholesterol(TC),glycerin three greases(TG),fasting blood glucose,glycosylated hemoglobin(HbA1C),low density lipoprotein cholesterol(LDL-C),2 hour postprandial blood glucose,apolipoprotein alpha(Apo(a)),blood routine.Retrospective analysis was made on the risk factors of diabetic retinopathy.Result1.After physical examination and laboratory testing,there were 176 cases of DR patients in the total of 529,158 cases NPDR patients and 18 cases PDR patients in the total of 176.The Prevalence rate of DR is 33.27?.The Prevalence rate of NPDR and PDR was 29.87?? 3.40?.2.There was significant difference between DR patients and NDR patients in distribution in age,duration of diabetes,hypertension,coronary heart disease,diabetic nephropathy,history of hypoglycemia and hyperuricemia(P ? 0.05);There was significant difference between DR patients and NDR patients in LDL-C,blood glucoses and levels of blood pressure(P?0.05).3.Logistic regression analysis of single factor indicated,there was significant difference between DR patients and NDR patients in age,duration of diabetes,LDL-C,HbA1 C abnormalities,hypertension,combined CHD,diabetic nephropathy,history of hypoglycemia and hyperuricemia(P?0.05).OR(95%CI)were 1.66(1.56,2.39),2.04(1.21,2.66),1.75(1.32,2.33),2.08(1.11,2.76),1.98(1.37,2.85),1.69(1.15,2.52),1.82(1.20,2.77),1.70(1.18,2.45)and 2.38(1.38,4.12).4.Logistic regression analysis of multiple factor indicated,there was significant difference between DR patients and NDR patients in age,duration of diabetes,LDL-C,HbA1 C abnormalities,hypertension,combined CHD,diabetic nephropathy,history of hypoglycemia and hyperuricemia(P?0.05),OR(95%CI)were 1.53(1.13,2.07),1.77(1.14,2.56),1.57(1.29,2.36),1.96(1.14,2.53),1.92(1.15,2.61),1.56(1.12,2.17),1.79(1.19,2.68),1.81(1.20,2.72)and 1.77(1.24,2.54).Conclusion1.The prevalence rate of diabetic retinopathy in patients was the same as the general population.2.There was significant difference between DR Patients and NDR Patients in age,duration of diabetes,,hypertension,combined CHD,diabetic nephropathy,history of hypoglycemia and hyperuricemia3.In the factors that affect DR,age,duration of diabetes,LDL-C,HbA1 C,hypertension,combined CHD,diabetic nephropathy,hyperuricemia and history of hypoglycemia may be the risk factors.It can effectively prevent and control the occurrence and development of DR,and improve the treatment effect and quality of life of the patients after taking intervention measures.
