Objective: The purpose of this study was to treat patients with diuretic-resistant chronic heart failure(CHF)who suffered from qi-deficiency and blood-stasis and yang deficiency.The study was based on conventional western medicine plus Yi Qi Wen Yang,Huo Xue Li Shui method combined with acupoint application and Ginger moxibustion,observing its effect on CHF diuretic resistance in patients with clinical symptoms,heart function(BNP,echocardiogram,6MWD,NYHA),symptoms of TCM,quality of life in Minnesota and exploring a new approach to the diuretic resistance of chronic heart failure with integrated Chinese and Western medicine.Methods: 100 patients with diuretic resistance who met the inclusion criteria were randomly divided into control group and treatment group with 50 cases each.The control group was treated with conventional Western medicine.The treatment group was treated with Yi Qi Wen Yang,Huo Xue Li Shui method combined with acupoint application and ginger-separating moxibustion on the basis of the treatment program of the control group for 14 days.The clinical symptoms and cardiac function before and after treatment were observed and compared between the two groups of patients.(BNP,echocardiography,6MWD,NYHA),TCM symptom scores,and Minnesota Quality of Life(MLHFQ)scores to explore the efficacy and safety of traditional Chinese medicine for Yi Qi Wen Yang,Huo Xue Li Shui method for the treatment of patients with chronic heart failure diuretic resistance,clinical The treatment of such patients provides new ideas.result:1.There were differences in the indexes of cardiac ultrasound before and after treatment between the two groups,among which the difference of LVEF and LVED in the control group was significant(P<0.05),the difference of SV,CO,LAD was very significant(P<0.01),The difference of the LVEF,LVED,SV,CO and LAD was very significant(P<0.01)in the treatment group.,suggesting that both groups of treatment could improve the patient’s echocardiographic indicators;there was no difference between the two groups after treatment(P>0.05).2.BNP,6MWD,body weight,24-hour urine output,creatinine,and uric acid levels in the two groups before and after treatment were all significantly different.Among them,BNP,6MWD,and uric acid were significantly different in the control group(P<0.01).Body weight,urine volume,and creatinine The difference was significant(P<0.05).The difference in BNP,6MWD,body weight,24-hour urine volume,creatinine,and uric acid in the treatment group was significant(P<0.01),suggesting that both groups of treatment can improve patients with BNP,6MWD,body weight,24-hour urine The levels of serum creatinine and uric acid were significantly different between the two groups after treatment.There was a significant difference in 6MWD,body weight,creatinine,and uric acid(P<0.05).BNP and urine output were significantly different(P<0.01).3.The efficacy of 6MWD and NYHA after treatment in the two groups was significantly different after the rank sum test(P<0.05).4.There were differences in the individual TCM symptom scores before and after treatment and the TCM syndrome total scores before and after treatment.Among the two groups of patients,palpitations,chest tightness(pain),shortness of breath,cough,chills,cold limbs,fatigue,bloating,edema of limbs,spontaneous sweating,hot drinks,and TCM symptom scores were significantly different(P< 0.01);between the two groups of patients after treatment,palpitation,chest tightness(pain),asthma,aversion to cold,fatigue,fatigue,spontaneous perspiration,TCM syndrome scores were significantly different(P <0.05),facial edema was very significant difference(P<0.01),there was no difference in cough,bloating,and hot diet(P>0.05).5.There were significant differences in the MLHFQ group before and after treatment between the two groups.There were significant differences in the physical status,emotional status,and other conditions in the control group(P<0.05).The physical status,emotional status,and other conditions in the treatment group were significantly different(P<0.01).There was significant difference in physical condition between the control group and the treatment group(P<0.05),and there was no difference in emotional status and other conditions(P>0.05).6.Liver function(ALT,AST)before and after treatment in the two groups were compared(P>0.05),and there was no statistical significance;within three days after treatment,electrolytes(Na+,Ka+)were compared within the two groups,in which two groups of serum sodium and pretreatment phase.Ratio,P>0.05,no statistical difference;blood potassium in the two groups compared with before treatment,serum potassium in the control group,P <0.01,the difference was significant,the serum potassium in the treatment group,P> 0.05,no statistical difference;There was a significant difference between the two groups of patients in the potassium group after treatment,P<0.05.conclusion:1.Both groups of treatment can improve cardiac function,reduce BNP levels,increase LVFF values,stroke volume and cardiac output,and improve the 6-minute walk distance and NYHA functional class,which is reduced in BNP levels,increased by 6 minutes For the distance of walking test and NYHA functional class,the efficacy of TCM-WM combined group was better than that of control group.2.The two groups of treatment can improve the patient’s sodium water retention and renal function,in the reduction of weight,increase urine output,reduce creatinine,uric acid levels,the efficacy of the combination of traditional Chinese and Western medicine group was better than the control group.3.Both groups of treatment can improve the clinical symptoms and TCM symptoms of patients.The efficacy of TCM-WM combined group is better than that of the control group.4.Both groups of treatment can improve the quality of life of patients with Minnesota(MLHFQ)score,in which the physical condition of the Chinese and Western medicine group is better than the western medicine group.5.Yi Qi Wen Yang,Huo Xue Li Shui method has better safety,and the incidence of hypokalemia in TCM-WM group is lower than that of western medicine alone.
