Objective: to evaluate the safety and efficacy of coronary artery bypass grafting(CABG)combined with coronary endarterectomy(artery bypass graft coronary artery endarterectomy)in the treatment of chronic total coronary artery occlusion(CCAO).Methods: the clinical data of 72 patients with coronary artery bypass grafting and coronary endarterectomy were retrospectively analyzed from January 2013 to August2017.The patients were followed up for 2 years and the preoperative and postoperative parameters were observed and compared.The patency rate of coronary artery and the accident of cardiovascular and cerebrovascular diseases after operation were summarized and the operative effect of CABG CE was summarized.Methods: the clinical data of 72 patients with coronary artery bypass grafting and coronary endarterectomy were retrospectively analyzed from January 2013 to August2017.The patients were followed up for 2 years and the preoperative and postoperative parameters were observed and compared.The patency rate of coronary artery and the accident of cardiovascular and cerebrovascular diseases after operation were summarized and the operative effect of CABG CE was summarized.Results: among all the 72 patients undergoing coronary artery bypass grafting and endarterectomy,34 cases had anterior descending branch,22 cases had right coronary artery trunk,4 cases had diagonal branch,6 cases had posterior descending branch,and 6 cases had blunt marginal branch.2 cases died during perioperative period,other patients recovered well early after operation,and NYHA grade was improved from preoperative ?-? to postoperative ?-?.Left ventricular end diastolic diameter(left ventricular end-diastolic dimension LVEDDD)and left ventricular ejection fraction(left ventricular ejection fraction),left ventricle ejection fractionation(LVEF)at 24 months after operation were better than those before operation.In 15 patients who underwent double source CT reexamination,the internal mammary artery grafts were patency without stenosis 1 year after operation,6 cases had slight stenosis after 2years of venous graft,8 cases had moderate stenosis after 2 years of coronary artery stripping.The incidence and mortality of cardiovascular and cerebrovascular accidents were lower than 3 in 2 years follow-up.Conclusion: coronary artery bypass grafting combined with endarterectomy is an effective treatment for patients with chronic total coronary artery occlusion.Improvement of early quality of life and reduction of perioperative mortality were positive.According to the follow-up data of 15 patients with coronary CTA,8 patients with moderate stenosis after 2 years of coronary artery exfoliation were found.The time of re-stenosis in coronary artery stripping is relatively short,so the indication of controlled intimal stripping should be strictly determined.
