ObjectiveIn this study we have used ultrasound transient elastography to exam the hepatic elasticity in patients with obstructive jaundice,and quantitatively explored the relationship between the degree of jaundice and elasticity,yet we established a mathematical model for providing a more accurate guidance for clinical diagnosis.Methods23 patients with obstructive jaundice who were admitted to the First People's Hospital of Yunnan Province from October 2016 to January 2018 were selected and excluded from hepatitis,liver parasitic disease,liver neoplasms,autoimmune liver disease,etc.Those patients with obstructive jaundice were classified into three categories:light,moderate,and severe.According to our plan,51 patients without jaundice at the same time were selected as the control group,and the history of liver disease was also excluded.Using ultrasound transient elastography,the above patients selected were tested for liver elasticity,and the data were analyzed by statistical.Finally,the relevant mathematical model was obtained to clarify the relationship between the level of jaundice and the elasticity value,thus initially clarifying the clinical using of the technique for patients with obstructive jaundice.ResultsThe liver stiffness values??of the non-jaundice group and the jaundice group were analyzed by the non-parametric rank sum test(U test).The average ranks of the two groups were 26.0 and 63.0,P<0.001,and the difference was statistically significant.Then,the three components of the Astragalus membranaceus were light,moderate,and severe.The non-parametric rank sum test(Kruskal-Wallis test)was performed on the non-jaundice group.The average rank of liver stiffness values??was 56.5,65.5and 72.0,P<0.001,the difference was statistically significant.In order to clarify the differences between the groups,the comparison between the four groups were further performed.The results showed that the differences between the groups were statistically significant.Finally,the relationship between liver stiffness measurement and jaundice grade was explored.We used rank correlation analysis(Spearman test)for exploring and we found that the correlation coefficient between liver elasticity and jaundice grade as following:r=0.6,P<0.001,indicating that there is a positive correlation between liver stiffness measurement and the degree of jaundice.The data were plotted with a scatter plot and linear regression analysis was performed to obtain a linear regression equation for the fit R~2 linear=0.958,y=-85.4+26.7x(x stand for total birubin and y liver stiffness).The receiver operating characteristic curve(ROC)validates the effectiveness of transient elastography in the diagnosis of jaundice.ConclusionsQuantification of obstructive jaundice by ultrasound transient elastography revealed that there was a positive correlation between the degree of jaundice and the value of liver stiffness.A linear regression model was initially obtained,which would provide a possibility for clinical non-invasive evaluation of jaundice in patients with obstructive jaundice.It can be used as an indicator to evaluate the liver function of patients with obstructive jaundice.It is expected to provide a more accurate guidance for clinical diagnosis and treatment of the disease,therefor it was worthing further investigation in clinic experense.
