Objects Breast cancer is one of the most common malignant tumor in women.In the whole process of dia,gnosis,treatment and prognosis of malignant tumor,the mental and physical conditions of breast cancer patients were born variety of problems.Therefore,the objects of this study are to measure the depression,anxiety and body image problems of breast cancer patients,to analyze the correlation between their emotional status and body image,immune index,quality of life.And further to explore the effects of group cognitive behavioral therapy(GCBT)on the depression,anxiety,body image,quality of life and immune index of breast cancer patients to prove the effectiveness of GCBT through clinical practice.Methods Patients with breast cancer in Department of Breast Surgery,Zhongda Hospital affiliated to Southeast University and through WeChat recruitment advertisements were selected to be samples.The patients were divided into group therapy group and control group.21 in the group therapy group and 3 dropped out after 8 weeks.20 in the control group and 12 dropped out after 8weeks.Subjects in the therapy group were received GCBT in addition to regular reexamination in hospital.The intervention was implemented for 60-90 minutes every week for 8 weeks.Patients in control group were only given regular reexamination.The Patient Health Questionnaire-9(PHQ-9),Generalized Anxiety Disorder-7(GAD-7),Body Image after Breast Cancer Questionnaire(BIBCQ),Functional Assessment of Cancer Therapy-Breast(FACT-B)and immune index(IL-6)were used to evaluate the effectiveness of GCBT.Results 1.The levels of depression and anxiety prior intervention in patients with breast cancer were 11.15±4.38 and 10.29±3.35.The depression was significantly positive correlation to anxiety.The depression was significantly positive correlation to body image,limitations,arm concerns and negative correlation to quality of life,physical well-being,additional concerns.The anxiety was significantly positive correlation to transparency,the level of IL-6 and negative correlation to physical well-being,additional concerns.2.There was a significant decrease in scores of PHQ-9 in comparison with baseline and the fourth week measures of the intervention group(F=5.32,P<0.05);There was a significant decrease in scores of GAD-7 in comparison with baseline and the fourth week measures of the intervention group(F=12.73,P<0.05);There were significant decrease in scores of BIBCQ,vulnerability scale,limitations scale,transparency scale in comparison with baseline measures of the intervention group(t = 5.09,t=2.34,t=2,22,t= 2.31,all P<0.05).There were significant increase in scores of FACT-B,social family well-being and emotional well-being in comparison with baseline measures of the intervention group(t=-2.20,t =-4.23,t =-3.78,all P<0.05);There was a significant decrease in level of IL6 in comparison with baseline measures of the intervention group(t=3.35,P<0.05).3.There was a significant difference between the group therapy group and control group in the subtraction scores of PHQ-9(t=-3.16,P<0.01);There was a significant difference between the group therapy group and control group in the subtraction scores of GAD-7(t=-4.79,P<0.01);There were significant differences between the group therapy group and control group in the subtraction scores of BIBCQ and vulnerability scale(t =-2.23,t=3.79,all P<0.05);There were significant differences between the group therapy group and control group in the subtraction scores of FACT-B,social family well-being and emotional well-being(t =2.11,t =3.22,t = 2.97,all P<0.05).ConclusionsPatients with breast cancer have obvious depression,anxiety,body image problems,decreased quality of life and damaged immune function.GCBT could effectively improve the psychological status of breast cancer patients.GCBT could improve breast cancer patients' depression and anxiety,ease their body image problems and improve the quality of life.GCBT could significantly enhance the immune function of breast cancer patients.
