Objective:To explore the correlation between the dynamic changes of sCD62 E,sCD14,vWF and APACHE ?score,SOFA score in patients with sepsis,and to analysis the severity,the therapertic evaluation and the prognostic significance of combined determinations of sCD62 E,sCD14,vWF in patients with sepsis in early stage.Methods:50 patients including sepsis group(n=36)and noinfection group(n=14),and another control group(n=22)whose physical examinations were normal in Critical Care Medicine(ICU)of the First People's Hospital of TaiCang in Suzhou from September 2015 to June 2016 were admitted in the study.The peripheral blood samples were collected on the 12 th,24th,72 nd hour after admission to measure serum sCD62 E,sCD14,plasma vWF by means of ELISA and APACHE ?score,SOFA score were recorded at the same time.The data was analysised by SPSS19.0.Result:1.The expressions of the serum soluble E-selectin(sCD62E),soluble cluster of differentiation 14(sCD14)levels and the plasma von Willebrand Factor(vWF)levels dramatically increased in patients with sepsis than those in the nonsepsis group(P<0.01).The expression levels of sCD62 E,sCD14 and vWF were significantly increased in sepsis-shock group than those in the nonsepsis-shock group.It may suggest that the dynamic changes of s CD62 E,sCD14 and vWF can be used as the indexes for occurrence,development and pathogenetic condition changes of sepsis.2.The levels of sCD62 E,sCD14 and vWF showed the significant positive correlation with SOFA score(P<0.05 or P<0.01);the levels of sCD14 showed the significant positive correlation with APACHE ? score(P<0.05).These results indicated that the levels of sCD62 E,sCD14 and vWF reflected the pathogenetic condition and the organ function.3.The levels of sCD14 on the 12 th hour after admission were significantly higher in the death group than those in the survival group(P<0.05).It may suggest that the levels of sCD14 in patients with sepsis have an advantage in the aspect of assessing.4.The specificity of sCD62 E,sCD14,vWF and the sensitivity of sCD62 E,sCD14 were high,the sensitivity and the specificity of joint detection with sCD62 E,sCD14,vWF were better than the sensitivity and the specificity of indidual indicator detection,and these findings indicated that there were more early estimated values of joint detection with sCD62 E,sCD14,vWF than those of indidual indicator detection in patients with sepsis.Conclusions:The results of this study preliminarily showed that the damage of EC,the disturbance of blood coagulation and the immunoreaction play an important role in the process of sepsis.The levels of sCD62 E,sCD14,vWF in the peripheral blood were positively correlated with progression of sepsis.The levels of sCD14 in patients with sepsis have a better predictive value about their assessment.The levels of sCD62 E,sCD14 may be considered as one of the independent risk factors for sepsis,so,they may have the advantages in predicting the prognosis of patients with sepsis shock.There are more early estimated values of joint detection with sCD62 E,sCD14,vWF than those of indidual indicator detection in patients with sepsis.
