Research backgroundBenzathine penicillin,also known as long-acting penicillin,is a white crystalline powder of dibenzyl ethylenediamine salt mixed with suspension and buffer.Benzathine penicillin has good effect to inhibit bacterial cell wall synthesis,and thus to kill bacteria.Benzathine penicillin is stored in the injection site,then slowly released and hydrolyzed to penicillin,so the effect is the same as penicillin.Benzathine penicillin action time longer,but due to the low produce the concentration of the penicillin,therefore applies only to microbial infection is highly sensitive to penicillin,treponema pallidum is sensitive to penicillin,so as the preferred treatment for syphilis benzathine penicillin drugs.Because penicillin is very difficult to dissolve in water,and it is easy to cause needle clogging,it is not conducive to the application of intravenous drip,and intramuscular injection is regarded as the preferred method of medication in the clinical treatment process.However,benzathine penicillin granule is large,which is soluble in water and forms a white emulsion,thick and thick,and the muscle injection often appears to be blocked by needles.Clinically often,in order to prevent the needle the blockage of coarse needle(9),a large dose of solvent(5 ml)injections and the rapid push note method,type of operation success rate is high but for patients with great pain and fear,and as a result of benzathine penicillin slowly absorbed,can increase the duration of pain at the injection site and induration formation rate.In the course of injection of benzathine penicillin,the patient chose to terminate the treatment because of the pain.Some patients received more pain than pain during injection,which led to dissatisfaction with the quality of the injection,resulting in decreased nursing satisfaction.Nursing staff are under the pressure,fearing that the failure and pain will lead to a dispute.Therefore,it is urgent to reduce the congestion rate,reduce pain,and reduce the formation of hard knot.ObjectiveBy intramuscular injection of benzathine penicillin method improved research,to explore the clinical quality of injection of benzathine penicillin,in order to relieve pain patients in the process of injection and injection after local pain,shorten the duration of pain,reduce the incidence of induration after injection,etc.To provide more reference.MethodChose to vistit The People's Hospital of Fuyang City from February 2015 to March2016,178 cases of 2.4 million U patients with benzathine penicillin injection were needed for this study.Using self-control method,benzathine penicillin was injected into both sides of the muscle and injected 1.2 million U per side.Routine group:300cases with left injection on the left side were given routine injection on 1/3 of the anterior superior iliac spine and the tailbone attachment.Modified group:300 cases with the right injection of the right side were modified intramuscular injection in the non-painful region of the gluteus.The success rate and the duration of the injection after injection,duration of pain and the occurrence of hard knot after injection were observed.ResultThe success rate was 99.0%(297/300)and the failure rate was 1.0%(3/300).In the routine group,257 cases were successfully injected,with a success rate of 85.7%(257/300),failure of 43 cases and 14.3%failure rate(43/300).the differences between groups was statistically significant(?~2=30.423,P<0.001);The degree of pain in the improved group injection was significantly lower than that in the conventional group,the difference was statistically significant(P<0.05).The pain duration of the modified group was significantly smaller than that in the conventional group,the differences between groups was statistically significant(P<0.05).There were 9 cases of hard knot in the modified group,and the incidence rate was 3.0%(9/300),and 29cases occurred in the conventional group.The incidence was 9.7%(29/300),the difference was statistically significant(?~2=9.900,P=0.002).ConclusionUsing multi-way joint control influence factors in the process of injection of benzathine penicillin,a injection can improve the success rate,and effectively reduce the patients pain intensity,pain duration after injection,the greatest degree to reduce the incidence of induration after injection;The improved method can greatly improve the injection quality of benzathine penicillin.
