Purpose:To explore the distribution of TCM syndromes and the relationship with insulin resistance in perimenopausal obstructive sleep apnea hypopnea syndrome(OSAHS),and to provide some basis for the clinical treatment of perioperative OSAHS patients.Method:1.108 patients with perimenopausal OSAHS were included in the study.Standardize the collection of patients with Chinese medicine four diagnostic information,the use of SPSS20.0 software to establish 108 patients with four diagnostic information entry database.Cluster analysis was performed using Ward s method and Squared Euclidean Distance in system clustering.According to the distribution of syndromes in the cluster,the expert type of the above-mentioned physicians of the three deputy physicians discussed the initial model of the cluster analysis and established the name of the syndrome.2.The sleep apnea hypopnea index(AHI)was monitored by cardiopulmonary bypass(CPC)detector.The levels of FSH,LH,E2 and FINS were measured by chemiluminescence method.The insulin resistance index(HOMA-IR)was calculated by hexose kinase assay.The patient’s height and weight were measured,and body mass index(BMI).The clinical indicators of each syndrome were compared and analyzed the clinical indicators of the syndromes and HOMA-IR correlation,and finally the HOMA-IR comparison of various syndromes.Results:1.Four diagnostic information distribution:Perimeter menopausal OSAHS frequency distribution of the top 10 symptoms:snoring,Shenpi fatigue,dizziness,sleepiness sleepiness,physical obesity,insomnia,Yaoxisuanruan,chest tightness,irritability,dry mouth and thirst.Tongue like red tongue,white greasy moss mainly.Pulse to the pulse,slip veins,veins,pulse-based.2.The four types of TCM syndromes were phlegm dampness syndrome,liver depression(Wang)spleen deficiency syndrome,phlegm and blood stasis syndrome,Qi and Yin deficiency syndrome.The distribution of syndromes was 31 cases(28.70%)of phlegm-dampness syndrome,28 cases(25.93%)of liver-stagnation and spleen-deficiency syndrome,24 cases(22.22%)of phlegm and blood stasis syndrome,and 18 cases(16.67%)of Qi and Yin deficiency syndrome.In addition,7 patients with TCM syndrome does not belong to the above four TCM syndromes.3.Comparison of clinical indicators of different TCM syndromes:Perimenopausal OSAHS different TCM syndromes age,FSH,LH no difference,P>0.05.The level of E2 in patients with deficiency of qi and yin is lower than other syndromes,the difference is statistically significant(P<0.05).The BMI of phlegm and blood stasis syndrome and phlegm stagnation syndrome was higher than that of deficiency of both qi and yin and spleen deficiency syndrome(P<0.05).The AHI of phlegm and blood stasis syndrome and Qi and Yin deficiency syndrome was higher than that of liver depression(wang)spleen deficiency syndrome,phlegm blocking syndrome,the difference was statistically significant(P<0.05).4.Correlation analysis:There was a positive correlation between BMI and HOMA-IR in phlegm-dampness syndrome group(r = 0.442,P<0.05).In liver depression(Wang)spleen deficiency syndrome group,E2 and HOMA-IR was negatively correlated(r =-0.487,P<0.05).There was a positive correlation between AHI,BMI and HOMA-IR in phlegm and blood stasis syndrome(r = 0.470,P<0.05;r = 0.518,P<0.05).There was a positive correlation between AHI and HOMA-IR(r = 0.494,P<0.05),and negative correlation between E2 and HOMA-IR(r =-0.471,P<0.05).5.Peri-menopausal OSAHS different TCM syndrome HOMA-IR average significant difference,with statistical significance,F=10.360,P<0.05.HOMA-IR of Qi-Yin Deficiency Syndrome and phlegm-blood stasis syndrome was higher than that of liver depression(wang)and spleen deficiency syndrome(P<0.05).Conclusion:1.Perimenopausal OSAHS most of the TCM syndromes can be identified as phlegm blocking syndrome,liver depression(Wang)spleen deficiency syndrome,phlegm and blood stasis cross-card,qi and Yin deficiency.2.Different TCM syndromes in the IR differences exist,Qi and Yin Deficiency Syndrome,phlegm and blood stasis syndrome patients with OSAHS and insulin resistance is significantly related to liver depression(Wang)spleen deficiency,deficiency of qi and yin Deficiency of patients with insulin resistance,phlegm blocking syndrome,phlegm and blood stasis each Conclusion Patients with overweight and insulin resistance.Qi and Yin Deficiency Syndrome,phlegm and blood stasis syndrome patients with the most severe insulin resistance.
