BackgroundThe updated staging definitions were presented at a meeting of over 400 professionals held in Chicago on April,2016.During the meeting,the participants also validated the new terminology using photographs.Pressure injury:a pressure injury is localized damage to skin and/or underlying soft tissue usually over a bony prominence or related to a medical or other device.The injury can present as intact skin or an open ulcer and may be painful.The injury occurs as a result of intense and/or prolonged pressure or pressure in combination with shear.The tolerance of soft tissue for pressure and shear may also be affected by microclimate,nutrition,perfusion,co-morbidities and condition of the soft tissue.The development of pressure injury can lead to pain and infection,affect the recovery of patients and prolong the time of hospitalization,which is one of the indicators that lead to poor prognosis.The stage of pressure injury is mainly based on the phased standard recommended by the American National Pressure Ulcer Advisory Panel(Appendix 1).The pressure injury can be divided into 1-4 stages,unstageable pressure injury and deep tissue pressure injury.The occurrence time of pressure injury in operation patients is usually within hours to 6 days after operation,and most of them are on the 1-3 day after operation.Our hospital is a large tertiary Grade-A comprehensive hospital.In the Department of orthopedics,260 patients with thoracolumbar posterior approach were treated each year so that it is easy to collect detailed information.Our hospital has a special nursing research team responsible for the guidance and funding of the project.The pressure ulcer management group is established to assess the preoperative skin of surgical patients and identify high-risk patients with pressure ulcers in the operation.Relative fixation of circuit nurses in department of orthopedics in the operating room is in order to facilitate the collection of data and evaluation.ObjectiveTo explore the effect of Mepilex in prevention of pressure injury in posterior thoracolumbar spinal surgery.MethodsWe have collected a total of 120 patients who underwent posterior thoracolumbar spinal surgery as the research object.According to random mode,the patients were divided into observation group and control group.ResultsWith the treatment of the application of Mepilex dressing in 30 minutes after surgeries,the percentage of patients who got the pressure injury(stage 1)in the observation group was 5.00%,while in the control group,the percentage was as high as 18.33%(P<0.05).What's more,with the immediate use of Mepilex dressing in the first day after surgeries,the percentage of patients who got the pressure injury(stage 1)in the observation group was 1.67%,which was significantly lower than the percentage of 11.67%(P<0.05)in the control group.Stage 3 and 4 pressure injury is not occurred in both of the patients of the observation group and the control group.ConclusionTo prevent the occurrence of pressure injury in posterior thoracolumbar spinal surgery were studied,using Mepilex can play a preventive role to a certain extent,it is worthy of clinical promotion.
