Objectives:1.To observe the effect of early back muscle function exercise on the treatment of thoracolumbar compression fractures after PKP surgery2.To provide theoretical basis for early back muscle functional exercise after PKP surgery through the process of finite element analysis of CT data from one case of treatment groupMethods:1.The data were collected from thirty patients with thoracic and lumbar compression fractures who were treated in Yunnan Provincial Hospital of Traditional Chinese Medicine from November 2016 to February 2018 and these patients were divided into two groups(treatment group and control group),with 15 patients in each group.Patients in the treatment group began to exercise back muscle function exercise three days after PKP surgery.While,patients in the control group rested on the ded after PKP surgery,whoes circumference of the waist were fixed when they got out of the bed.VAS scores,ODI scores,JOA scores,and the height of the injured vertebrae column were measured in two groups of patients after PKP surgery and one month after surgery respectively to compare the treatment effects.2.Collecting CT data about lumbar vertebra from selected one case of treatment group before,three days after,and one month after PKP surgery repectively were to establish a finite elecment model and then accordlingly conduct the finite element analysis with the purpose of proving the effect of early postoperative back muscle function exercise after PKP surgery supported by comparing the data from the tree period of timeResults:1.Clinical efficacy observation:(1)VAS score: there is no statistical difference in VAS scores before and after surgery between the two groups(p=0.585,p=0.372).Specially,the data can be compared.The VAS score at 3 days after PKPwas lower than that before surgery,and the difference was statistically significant(p<0.05).The VAS scores at one month after surgery were significantly better in the treatment group than in the control group(p<0.05).(2)ODI scores: There was no statistical difference in ODI scores between the two groups before and after surgery(p=0.644,p=0.792).Specially,The data can be compared.The ODI score on the 3rd day after PKP was lower than that before surgery,and the difference was statistically significant(p<0.05).The ODI scores of the two groups in one month after operation were stistically different between the treatment group and the control group(p<0.05).(3)JOA score: There was no statisctial difference in JOA scores between the two groups before and 3 days after operation(p=0.789,p=0.543).Specially,the data can be compared.For the comparision of JOA scores after one month,the treatment group was superior to the control group and the difference was statistically significant(p<0.05).The improvement rate of JOA score at one month after PKP in both groups was better that in the treatment group than in the control group,and the difference is stastically significant(p<0.05).(4)Measuirng the height of injured vertebral column:There was no statistical difference in the vertebral height between the two groups before and after the PKP surgery(p=0.846,p=0.895).Specially,The height of the vertebral injury was significantly higher than that before the operation on the 3rd day after PKP surgery.The difference was statistically significant(p<0.05).There was no statistically difference in the height of injured vertebral column between the two groups in one month after operation(p=0.098).2.Finite element analysis: The results of finite element analysis show that as time goes by,the deformation of the L1?L2(injured vertebral column)?L3 as a whole and each part under the same load condition is generally decreasing,which proves the patient's surgical effect and recovery effect.Conclusions:1.PKP surgery can significantly edd the pain of patients and effectively restore the height of the injured vertebral column;2.The early effect of PKP for the treatment of thoracolumbar vertebral compression fractures after the exercise of the dorsum is effective,which can effectively reduce the patient's pain and improve the patient's postoperative quality of life.
