Purpose:To explore the application of 3D printing technology-assisted total knee arthroplasty and traditional total knee arthroplasty in clinical efficacy and safety of the comparative study. Methods:From January 2017 to 2018 have 10 patients who underwent initial total knee arthroplasty at the Shenyang Medical College Affiliated Hospital,Ten patients underwent total knee arthroplasty, one male, nine female, and ten patients underwent unilateral total knee arthroplasty, aged 61-80 years, with an average of 68)year old.Randomly divided into two groups, each group of 5 cases of knee replacement, the surgeon of the two groups of the same surgeon, postoperative routine analgesic, anticoagulant, proper functional training guide treatment. The experimental group used 3D printing technique to assist the total knee arthroplasty, while the control group received the traditional total knee arthroplasty. Two groups of age, gender, were not statistically different,The knee joint scores of patients with preoperative Hospitals(Hospital for Special Surgery,HSS)and knee HSS scores at 3 months postoperatively were recorded. The operation time, the total amount of bleeding(intraoperative blood loss + postoperative follow-up flow),Postoperative X-ray findings included the Orthographic knee Femoral flexion angle, tibia angle;lateral femoral flexion angle(femoral prosthesis anteversion angle),tibia angle(tibial prosthesis posterior tilt);postoperative infection.Result:3D printing personalized osteotomy component total knee arthroplasty group operation time(105.5 ±3.39)min,Traditional total knee arthroplasty group operation time(124.83±4.87)min,The difference between the two groups was statistically significant(t=-7.97 p=0.000).HSS score(48.83 ±3.13)in preoperative knee arthroplasty assisted by 3D printing personalized osteotomy kit and preoperative knee HSS score(48.5±1.22)in traditional total knee arthroplasty group, with no difference between the two groups Statistical significance(t=24 P=0.813).3D printing personalized osteotomy component total knee arthroplasty group blood loss(447.5±35.17)ml,Traditional total knee arthroplasty group blood loss(578.3±29.94)ml,The difference between the two groups was statistically significant(t=-6.93 P=0.000).3D printing personalized osteotomy component total knee arthroplasty X-ray of Orthographic knee Femoral flexion angle(96.33±0.81),Traditional total knee arthroplasty group(95.83±0.75),with no difference between the two groups Statistical significance(t=1.1 P=0.296).3D printing personalized osteotomy component total knee arthroplasty X-ray of Orthographic knee tibia angle(89.8±0.44),Traditional total knee arthroplasty group(89.4±0.89),with no difference between the two groups Statistical significance(t=0.89 p=0.4).3D printing personalized osteotomy component total knee arthroplasty group X-ray of lateral knee Femoral flexion angle(7.66±=1.63),Traditional total knee arthroplasty group(7.83±1.94),with no difference between the two groups Statistical significance(t=0.16 p=0.875).3D printing personalized osteotomy component total knee arthroplasty group X-ray of lateral knee tibia angle(86.33±0.81),Traditional total knee arthroplasty group(86.67±2.34),with no difference between the two groups Statistical significance(t=0 p=1.000).3D printing personalized osteotomy component total knee arthroplasty group knee HSS scores at 3 months postoperatively were(86,67±1.21),Traditional total knee arthroplasty group knee HSS scores at 3 months postoperatively were(86.63±0.51),with no difference between the two groups Statistical significance(t=0.15 p=0.880).3D printed personalized osteotomy component assisted lowering total knee arthroplasty group lower limb force line less than 4° ratio of 80%,In the traditional total knee arthroplasty group, the ratio of the lower extremities to less than 4° is 40%..3D printing personalized osteotomy accessory assisted total knee arthroplasty group preoperative knee HSS score(48.83±3.13),Three months after surgery, the knee HSS score(86.67±1.21),There are statistical differences(t=-27.65 p=0.000).Traditional total knee arthroplasty group preoperative knee HSS score(48.5±1.22),Three months after surgery, the knee HSS score(86.67±2.34),There are statistical differences(t=-35.18 p=0.000).Conclusion:Total knee replacement in the.3D printing personalized osteotomy component total knee arthroplasty group was shorter and less bleeding than Traditional total knee arthroplasty,A significant increase in knee HSS score after total knee arthroplasty confirmed the efficacy of total knee arthroplasty,.3D printing personalized osteotomy component total knee arthroplasty group and Traditional total knee arthroplasty at Postoperative X-ray of Orthographic knee Femoral flexion angle, tibia angle;lateral femoral flexion angle(femoral prosthesis anteversion angle),tibia angle(tibial prosthesis posterior tilt);postoperative infection is no difference between the two groups Statistical significance. However, the ratio of the lower limb force line of less than 4° for the 3D printed personalized osteotomy kit assisted lowering total knee arthroplasty group was better than the ratio of the lower limbs lower than 4° for the conventional total knee arthroplasty group. There was no statistically significant difference in the HSS score of the knee joint between the 3D printed individual osteotomy kit and the total knee arthroplasty group and the traditional total knee arthroplasty at 3 months after operation.
