| Objective:This study was conducted to collect and summarize the clinical data,clinical signs,sex hormones,ovarian morphology,and hemodynamics in patients with polycystic ovary syndrome(PCOS)of the syndrome of kidney deficiency and phlegm stasis syndrome,which is for the correction analysis between interstitial blood flow and body mass index in PCOS patients with kidney deficiency and phlegm stasis type.To evaluate the clinical effect of Ankun decoction combined with Daying-35,to explore the mechanism of its effect,and to provide the basis for clinical treatment.Methods:The information of gynecological outpatients with PCOS of kidney deficiency and phlegm stasis syndrome occurred from March 2017 to January 2018 were collected from gynecological and reproductive department of Jiangsu Integrated Chinese and Western Medicine Hospital.According to the order of visiting,they were randomly divided into treatment group and control group with 30 cases in each group.The clinical manifestations,physical signs,laboratory examination and B-ultrasound were recorded.The treatment group was treated with Ankun decoction plus Daying-35,while the control group was treated with simple Daying-35 for 3 consecutive cycles.Through statistical analysis and observation on the correlation between ovarian interstitial blood flow and other factors simulaneously,we compared the improvement of symptoms and signs before and after treatment in different groups and the changes of each index.Results:BMI,WHR,sex hormone index and pulsatile index(PI)and resistance index(RI)of interstitial artery of ovary were measured in 60 cases of PCOS patients with kidney deficiency and phlegm stasis.Sum:l.BMI and WHR were positively correlated with T,and there was significant difference between them(P<0.05).There was no significant correlation with LH and FSH(P>0.05).2.LH and T were negatively correlated with PI and RI of ovarian interstitial artery blood flow,and there was significant difference between them(P<0.05).There was no significant correlation with FSH(P>0.05).3.The blood flow PI and RI of the interstitial artery of ovary were negatively correlated with BMI and menstrual score,but not significantly with-WHR and course of disease.According to the results of curative effect,clinical symptom,biochemical index and B-ultrasound before and after treatment,we found that the total curative effect of the treatment group was higher than that of the control group,and there was significant difference between the treatment group and the control group(P<0.05).2.The curative effect of TCM syndrome in the treatment group is much better than that in the control group,and there was a very significant difference between the treatment group and the control group(P<0.01).3.1n clinical symptoms,there were significant differences in the improvement of BMI,WHR,acne and menstruation between the two groups(P<0.01).In the treatment group,the hirsutism symptoms of PCOS patients were improved to some extent with statistical difference(P<0.05),But there was no significant difference compared with the control group.At the same time,the treatment group was superior to the control group in improving acne and menstruation and reducing WHR(P<0.05).4.The changes of sex hormones in the two groups after treatment were lower than those before treatment,and there was significant difference between the two groups(P<0.01).After treatment,FSH was higher than that before treatment,and there was significant difference(P<0.05).The effect of the treatment group on reducing androgen level was better than that of the control group,and there was significant difference between the treatment group and the control group(P<0.05).There was no significant difference in LH and FSH levels between the two groups after treatment(P>0.05).5.There were significant differences in RI levels between the treatment group and the control group after treatment,and there was a very significant difference between the two groups in the changes of ovarian B ultrasound(P<0.01),PI in treatment group was significantly higher than that before treatment(P<0.05).There was no significant difference before and after PI treatment in the control group.There was no significant difference in PI and RI levels between the two groups after treatment.(P>0.05).Conclusion:1.The abnormal body weight and endocrine on the ovarian regional blood flow was positive.Ovarian regional blood perfusion directly affects the ovulation and menstruation of women.2.Ankun decoction plus Daying-35 can effectively improve the menstrual status and TCM symptoms of PCOS patients with kidney deficiency and phlegm stasis.It works better than the treatment with simple Daying-35.The effect of Ankun decoction on reducing androgen was more remarkable,and the effect of Ankun decoction on improving the hirsutism and acne was also better than that of the control group.3.Ankun decoction has important significance in improving ovarian blood flow perfusion and restoring the periodic changes of ovarian interstitial blood flow,which is worthy of our further study. |
