BackgroundType 2 diabetes mellitus(T2DM)is characterized by elevated blood glucose caused by insulin resistance and impaired insulin secretion.It is the most common chronic endocrine metabolic disease.Metabolism of human body's three substances is closely related,the abnormal metabolism of glucose is bound to affect the metabolism of fat and protein.The studies confirmed that T2DM patients were in the state of oxidative.The advanced oxidation proteinproducts(AOPP)is a new type of oxidative stress marker.There is little domestic and international research on the relationship between the AOPP and the body components of T2DM patients.In this study,the associations of serum level of AOPP with body composition and glucose metabolism in T2DM patients was investigated.ObjectivesBy detecting the serum AOPP level of T2DM patients and analyzing the body composition by Dual energy X-ray(DEXA),we discussed the correlation between AOPP and body composition,so as to provide a clinical basis for prevention and treatment of T2DM with obesity and sarcopenia.MethodsThis study is a cross-sectional study from Henan Province People's Hospital.A total of 186 subjects were enrolled,including 124 in group T2DM and 62 in control group.By asking the way to understand the subjects of history,course of disease,height,weight.Fasting blood specimens from serum samples were measured as routine biochemical indexes,AOPP was measured by enzyme-linked immunosorbent assay(ELISA),using the dual energy X-ray absorptiometry(DEXA)to measure the Android area of waist,Gynoid area of waist hip,upper limbs,lower limbs and Fat mass percentage(FM%)and the content of muscle.For the T2DM group according to the diagnostic criteria of obesity(BMI?25m~2)will be divided into obesity group and non obesity group,according to the Asian Sarcopenia Working Group(AWGS)diagnostic criteria for sarcopenia will be divided into sarcopenia group and non-sarcopenia group.All data were statistically analyzed by SPSS19.0.The difference of P<0.05 was statistically significant.Results1.T2DM group compared with the control group,the clinical features including HbA1c,FPG,2h-BG,TG,AOPP levels,T2DM group was significantly higher than control group(P<0.05),in terms of HDL-C and grip strength,T2DM group was significantly lower than control group(P<0.05);in the waist Android area FM%,lower limbs FM%and body FM%,T2DM group was significantly higher than in control group(P<0.05),in terms of muscle content in the waist Android area,the upper limbs FM%,muscle content in the lower limbs,and the body muscle content,the T2DM group was significantly lower than the control group(P<0.05).2.T2DM obesity group compared with non-obesity group,the clinical features including bodyweight,BMI,ASMI,T2DM obesity group was significantly higher than the non-obesity group(P<0.05),while the HDL-C level is significantly lower than the non-obesity group(P<0.05),there was no difference in AOPP between the two groups(P>0.05);body composition including the FM%of waist Android area,muscle content of waist Android area,muscle content of hip Gynoid area,muscle content of upper limb,muscle content of lower limbs and the body FM%and body muscle content,the T2DM obesity group was significantly higher than the non-obesity group(P<0.05).3.T2DM sarcopenia group compared with non-sarcopenia group,the clinical characteristics in body weight,BMI,grip strength,ASMI,T2DM sarcopenia group was significantly lower than non-sarcopenia group(P<0.05),the TG was significantly higher than the non-sarcopenia group(P<0.05),there was no difference in AOPP between the two groups(P>0.05);body composition including the muscle content of waist Android area,muscle content of hip Gynoid area,the upper and lower muscle content and body muscle content,the T2DM sarcopenia group was significantly lower than the non-sarcopenia group(P<0.05).4.The level of serum AOPP in T2DM group was positively related to duration HbA1c,2h-PG(P<0.05),but negatively related to the HDL-C(P<0.05);in the body composition,there was a positive correlation between the serum level of AOPP and FM%of waist Android area,upper FM%,body FM%,there was a negative correlation between the serum level of AOPP and the muscle content of the lower limb muscle(P<0.05).Conclusions1.The serum AOPP level of T2DM group was significantly higher than that of control group.2.The serum AOPP level in the T2DM patients is positively correlated with course of disease,FPG,2h-PG,and negatively correlated with HDL-C,showed the level of AOPP was associated with abnormal glucose and lipid metabolism in T2DM,and the severity of T2DM might be suggested.3.The serum AOPP level in the T2DM patients is positively correlated with waist area Android area FM%and whole body FM%,and negatively correlated with lower extremity muscle content,it is suggested that antioxidant stress therapy can improve the metabolism of fat and muscle tissue in T2DM patients and optimize the body composition of T2DM patients.
