BackgroundBenign prostatic hyperplasia is a common progressive urinary tract obstruction in the elderly.The main symptoms are increase nocturia frequency,frequent micturition and urinary insensitivity,which seriously affect the sleep,daily activities and quality of life of the elderly with BPH.With the aggravation of aging in China,the number of the elderly with BPH is increasing.Therefore,it is particularly important to control the development of lower urinary tract symptoms and relieve the suffering of the elderly with BPH.In view of this,this study aim to explore factors that affect the progression of lower urinary tract symptoms in the elderly with BPH,so as to facilitate control measures and improve their quality of life.ObjectiveTo explore the influencing factors of the progress of lower urinary tract symptoms in the elderly with BPH,and to provide theoretical basis for the effective intervention measures to control the development of their symptoms,so as to ease the pain of the elderly with BPH and improve their quality of life.MethodsIn Henan Province,12 living communities were stratified and clustered,and 300 the elderly with BPH who met the inclusion criteria were selected as the subjects.The research tools includes:?.general data questionnaire:age,BMI,educational background,how long does they have illness and so on.?.the life style questionnaire of the elderly with BPH:this questionnaire is a self-designed daily living habit questionnaire for the elderly with BPH.The questionnaire contains 21 items,includes drinking,diet,smoking,sports,sex life and so on.After a small sample survey,the Cronbach's alpha coefficient of the questionnaire is 0.764,the retest reliability is 0.712.?.the international prostate symptom score table(IPSS):IPSS is a commonly used clinical scale to determine the severity of lower urinary tract symptoms and quality of life in the elderly with BPH.?.the old age depression scale(GDS):the scale consists of 30 items,includes depression,reduction of activity,irritability,and withdrawal pain.The Cronbach's alpha coefficient of the scale is 0.846 and the retest reliability coefficient is 0.812.?.the self rating Anxiety Scale(SAS):SAS is a self rating scale,with a total of 20 items,and the 4 grade evaluation method is use.The Cronbach's alpha coefficient of the scale is 0.832.The data collected by SPSS17.0 were analyzed by variance and multiple linear regression analysis.Results1.The symptoms of lower urinary tract symptoms in the elderly with BPH In the elderly with BPH,mild symptoms were found in 32 elderly(10.67%),80 were moderate(26.67%)and 188 were severe(62.66%).The quality of life were 171(57%)of the elderly with BPH with distress and very bad.Among them,96(32%)were suffering from distress and 75(25%)were very bad.2.The lifestyle of the elderly with BPH Among the elderly with BPH,144(48.00%)had a drinking habit,of which 125(86.81%)drank liquor,78(54.17%)drank more than 1 times per day,and 83(57.64%)drank more than 40 years.In addition,102 elderly(34.00%)ate vegetables and fruits 2~3 times a day,and 271(90.30%)consumed meat as pigs and cattle.In daily exercise,219(73.00%)had a good habit of exercising,of which 113(51.60%)had less than 1 hours of exercise each time,and 174(79.45%)exercise were the form of walking.3.Depression and its influence on lower urinary tract symptoms in the elderly with BPH Among the elderly with BPH,there were 226(75.33%)without depression,74(24.67%)with depressive symptoms,46(15.33%)with mild depression and 28 in moderate to severe depression(9.34%).The lower urinary tract symptoms of the elderly with BPH with different depressive symptoms were statistically significant,F=3.312,P<0.05.There were a statistically significant difference in the lower urinary tract symptoms between the elderly with BPH without depression and those with moderate or severe depression,P<0.05.There were no significant difference in the lower urinary tract symptoms between the elderly with BPH without depression and those with mild depression and those with mild and moderate severe BPH,P>0.05.4.Anxiety and its influence on lower urinary tract symptoms in the elderly with BPH Among the elderly with BPH,there were 171 elderly without anxiety(57%),97 in mild anxiety elderly(32.33%),and 32 in moderate anxiety elderly(10.67%).There were a significant difference in anxiety status and lower urinary tract symptoms in the elderly with BPH,F=7.295,P<0.05.There were a statistically significant difference in lower urinary tract symptoms between elderly without anxiety and mild and moderate the elderly with BPH,P<0.05.There were no significant difference in lower urinary tract symptoms between mild anxiety elderly and moderate anxiety in the elderly with BPH,P>0.05.5.Single factor analysis of the influence of general condition and living habits on lower urinary tract symptoms in the elderly with BPH Age,marital status,monthly income per person,course of BPH,hypertension,tea,drinking,frequency and duration of drinking,spicy food,meat type,frequency of eating vegetables and fruits,exercise,frequency of exercise,time of exercise,form of exercise,and the number of times of sexual life(month).There were statistically significant differences in the lower urinary tract symptoms among other factors,P<0.05.6.Multiple regression analysis of factors affecting lower urinary tract symptoms in the elderly with BPH Age,marital status,monthly income per person,BPH course,frequency of drinking,duration of drinking,meat type,frequency of eating vegetables and fruits,frequency of exercise,time of exercise,form of exercise and anxiety were the regression equations of the lower urinary tract symptoms of elderly BPH patients as independent variables,The variation of the lower urinary tract symptom 55.70% was explained.The explanatory volume of each independent variable after the regression model reached a significant level,F=9.825,P<0.05.ConclusionsLower urinary tract symptoms in most the elderly with BPH were in severe condition.Increase of age,increase of monthly income per person,growing course of BPH,increase of anxiety and non married marital status were the aggravating factors of lower urinary tract symptoms.Appropriate frequency(3~4 times/week)and duration(more than 30 years)of drinking,higher frequency(2~3 times/day)of eating vegetables and fruits,proper frequency(3~4 times/week)and time(1~2 hours/day)of exercise,the type of meat of chicken and duck,and the form of ball/jogging/fast walking were protective factors of lower urinary tract symptoms.
