| Objective:To observe clinical effects of the acupuncture therapy of type 2 diabetes mellitus with coronary heart disease with the method of purging stomach,invigorating spleen and regulating heart.To discuss the effects of the blood glucose,blood fat and mechanism action of improving cardiac function from acupuncture therapy with the method of purging stomach,invigorating spleen and regulating heart.In order to provide basis for treatment of type 2 diabetes mellitus with coronary heart disease.Methos:62 patients who met the requirements were randomly divided into acupuncture group and comparison group by using random number table method,with 30 patients in acupuncture group and 32 patients in comparison group.All the patients were educated with personal health,low-salt and low-fat diabetes diet,non-smoking and little alcohol before joining the group.Everyone was required to do some moderate sports every day before feeling tired.At the same time,the original treatment of Western medicine remained unchanged,including the categories and dose of the medicine.Sub-lingual nitroglycerin was administered When angina acute attack.Based on the principle of purging stomach,invigorating spleen and regulating heart,acupuncture group was treated with acupuncture on the basis of the comparison group.The points are Zhongwan,Zusanli,Neiting,Pishu,Sanyinjiao,Taibai,Xinshu,Geshu,Neiguan,Shenmen,Danzhong.The depth of needle should rely on feeling qi coming.First stabbings are on Pishu,Xinshu,Geshu,then surplus points.No needle required after acupuncture.Zhongwan,Zusanli,Neiting,Geshu and Danzhong use diarrhea,with the first reducing method of lifting and thrusting,then twirling needle 6 times.Pishu,Taibai,Sanyinjiao,Xinshu,Shenmen and Neiguan use reinforcing method,with the first mentioned interpolation of lifting and thrusting,then twirling needle 6 times,reducing needle for 30 min.This treatment kept 3 times a week(One day at intervals)in 1 months,last three courses.All the moments and processes were dynamically tracked and researched,as well as the evaluation of the effects at the end of the treatment course.We also took notes of TCM symptom score,quality of life and health level,fasting blood glucose,glycated hemoglobin,postprandial 2H blood glucose and blood lipid(TC、TG、HDL-C、LDL-C),angina pectoris grade,electrocardiogram and so on,evaluating curative effect.Results:1.Through the contrast of overall effectiveness from two different groups,acupuncture group has the effectiveness rate of 82.1%,which is better than comparison group whose effectiveness rate is 54.8%.The result is statistically significant(P<0.05).2.All the groups had their TCM symptom scores dramatically decreasing after treatment(P<0.01),which has a statistically significant result compared with that before treatment.Acupuncture group has the effectiveness rate of 89.3%,while comparison group only has 61.3%.The result is statistically significant(P<0.05).And the symptom scores of the acupuncture group were significantly lower than that of the comparison group(P<0.05),especially in diabetes symptoms of polydipsia,polyuria and polyuria,CHD symptoms of chest tightness and chest pain and emotional impatience.3.Compared with what happened before treatment,all the groups helped improve the FPG,2hPG and HbAlc.The result is statistically significant(P<0.01).Acupuncture group was much better than comparison group(P<0.05).4.Compared with what happened before treatment,the total cholesterol,triglyceride,high density lipoprotein cholesterol and low density lipoprotein cholesterol in the two groups were improved.The result is statistically significant(P<0.01).TC and TG in acupuncture group decreased more apparently than that in comparison group,HDL-C in acupuncture group increased.The result is statistically significant(P<0.05).But comparison of LDL-C in two groups,the result is not statistically significant(P>0.05).5.After the treatment,acupuncture group has 85.7%of effectiveness on angina pectoris,which is better than that in comparison group who has 61.3%.The result is statistically significant(P<0.05).Acupuncture group has 82.1%of effectiveness on electrocardiogram,which is better that in in comparison group who has 52.1%.The result is statistically significant(P<0.05).6.After two groups of treatments,the life quality and health level of patients were all improved.The result is statistically significant(P<0.01).And the acupuncture group decreased more apparently than that in comparison group.The result is statistically significant(P<0.05).Conclusion:Acupuncture therapy of type 2 diabetes mellitus with coronary heart disease with the method of purging stomach,invigorating spleen and regulating heart has successfully improved the clinical symptoms of the patient and corrected the disorder of glycolipid metabolism,reduced the frequency and degree of angina pectoris,in order to improve the blood supply of coronary artery,postpone the process of coronary heart disease,also help patients improve their life quality and health level.The research finally reveal that acupuncture therapy has a really satisfactory effect on curing type 2 diabetes mellitus with coronary heart disease,whith is safety and reliably,and is worthy of clinical application. |
