Effect Of Intra-articular Injection Of Sodium Hyaluronate Combined With Aerobic Training On Knee Joint Cartilage In Rats

Objective: By injecting intra-articular sodium hyaluronate combined with aerobic training to KOA model rats,observe changes in articular cartilage and chondrocytes,Aggrecan and collagen type ? in articular cartilage.According to the above data,observe the effect of sodium hyaluronate injection on KOA and determine whether sodium hyaluronate combined with aerobic training is better.Methods : 1.Experimental Subjects: 40 SD male rats were randomly divided into four groups: control group,model group,SH group,sodium hyaluronate and aerobic training group.The control group was only housed in cage,without any intervention;the other three groups were induced to osteoarthritis model by the combination of papain and L-cysteine.After 4weeks,the SH group was treated with simple intra-articular injection and the sodium hyaluronate and aerobic training group was treated with intra-articular injection and aerobic training.2.Specific interventions: model group: cage feeding,free diet and activities,without any intervention.SH group: intra-articular injection of sodium hyaluronate 0.02 ml per time,once a week.Sodium Hyaluronate and Aerobic Training Group: Aerobic training began the next day after sodium hyaluronate injection(moderate intensity running training at 20 m /min for 30 minutes,daily,6 times per week).After continuous treatment for 4weeks,all the rats were killed by the tail-end and neck-cutting method,and the experimental articular cartilage was made to make specimens to observe the change of the cartilage and each index.3.Observations:(1)Macroscopicobservation of gross morphology of the articular cartilage in rats;(2)Changes of cartilage matrix and cartilage cells under HE staining;(3)Aggrecan and type? in cartilage by immunohistochemistry Collagen expression.(4)Statistical analysis.Results:1.Macroscopic of each group of knee cartilage: In control group cartilage surface was smooth,bright in color,no cracks,softening and joint effusion.In the model group,the surface of cartilage was rough,dull in color,cracked,fissured and eroded.Some of the specimens showed hyperplasia and osteophyte formation at the joint edge.In SH group cartilage surface was slightly uneven,light yellow in color,no obvious cracks,erosion and effusion.In Comprehensive group cartilage surface was basically flat,bright in color,no cracks,softening and effusion,with Occasionally mild congestion of synovial.According to the modified Mankin's score,the statistical results were ranked by sum test.Compared with the model group,sodium hyaluronate and Comprehensive group were statistically significant(P<0.05),in which sodium hyaluronate Aerobic training group improved more obviously.2.HE staining: In the control group,cartilage stroma stained evenly,the chondrocytes arranged neatly with same number and shape,the tide was complete.In the model group,cartilage layer became very thin,the staining of cartilage stroma was significantly reduced,the number of chondrocytes reduced in number and arranged disorganized,and the tide line was incomplete and disordered.In the SH group,the cartilage layer became thin slightly,chondrocytes slightly reduced in number and arranged relatively neatly,only one or two tide lines were incomplete.In the Comprehensive group,cartilage surface was smooth,the cartilage layer was basically uniform,the stroma evenly stained,the number and distribution of chondrocytes in each layer are more uniform,the tide line was basically complete.According to the modified Mankin's score,the results of HE staining were analyzed.Compared with the model group,sodium hyaluronate and Comprehensive group were statistically significant(P <0.05),in which sodium hyaluronate Aerobic training group improved more obviously.3.Immunohistochemistry results: Under the light microscope,the positive expression of type ? collagen was largely dyed blue.Aggrecan positive expression of large cartilage matrix was dyed brown.In the model group,the expression of Aggrecan and type ? collagen was weakly positive.The control group,SH group and Comprehensive group all expressed obviously.Compared with model group,the expression of Aggrecan and type? collagen in SH group and Comprehensive group had statistical difference(P<0.05),while compared with SH group,the expression of Comprehensive group also had statistical difference(P<0.05).Conclusion: Aerobic training can promote the synthesis of sodium hyaluronate on cartilage matrix type ? collagen and aggrecan.
