Objective:To evaluate the effectiveness?safety and clinical utility of cryopreserved platelets.By comparing the infusion curative effect of transfusion of manual concentrated platelet,apheresis platelet and cryopreserved platelets.To explore frozen platelet preservation liquid Dimethyl sulfoxide(Dimethyl Sulphoxide,DMSO)effects on the human bodyMethods:1.Retrospectively analyzed the efficacy and adverse reactions of 319patients who had platelets transfused in the Affiliated Hospital of Southwest Medical University from 2016.1-2017.6.30.The patients were divided into group 1(apheresis platelets group,105 cases),group 2(manual concentrated platelets group,102 cases),group 3(cryopreserved platelets group,112 cases).We recorded patient's basic information,disease diagnosis,the total number of platelets and the frequency of infusion,the platelet count before transfusion,and the platelet count in 24 hours after infusion.We managed the information about the changes of biochemical index(ALT,AST,TP,ALP)before and after platelets transfusion and the information of adverse transfusion reactions as well.We recorded patients clinical data(general examination,internal medicine,surgery,ophthalmology,otolaryngology,nervous system)before and after the infusion of cryopreserved platelets 4.We recalled 50 patients who had transfused cryopreserved platelets,to do physical examination,and detected biochemical indicators(ALT,AST,TP,ALP).According to the patient's discharge time,the patients were divided into half a year group,and one year group,we compared and the clinical data and statistical analysis with those before the infusion of frozen platelets.At last,we compared the case data,did the statistical analysis(choosing t-test for data obey normal distribution,rank sum test for date doesn't obey the normal distribution,and chi-square test for count date).Results:1.Platelet counts were significantly increased in three groups of patients.Three groups of appreciation on platelets were14.2(4.0,44.5)?12.0(1.0,28.0)?6.0(1.0,15.0).The differences between the three groups were statistically significant(Z=13.171,P=0.001),Group 3 platelets appreciated less than group 1 and 2;The percentage of cases of increased platelet count is 88.6%?84.3%?82.1%,there was no statistically significant difference in platelet rising percentage of three groups(X~2=1.800,P=0.407).2.The 24 hCCI were 9.6(2.1,18.4)?9.6(0.9,20.3)?4.5(0.7,9.22)The differences between the three groups were statistically significant(Z=15.288,P<0.001),the differences between group 1 and group 3,group 2 and group 3 were statistically significant(P<0.05),there was no statistically significant difference between the group 1and group 2(P=0.659).The 24 h CCI in group 3 was lower than group 1 and 2.3.The efficacy of platelet clinical infusion were 65.7%?62.7%?50.9%,and there was no significant difference among the three groups(X~2=5.574,P=0.062).The adverse transfusion reaction in group 1 is(8,7.4%,)in group 2 is(14,13.3%),and in group 3 is(5,4.5%).The difference were statistically significant(X~2=6.054,P=0.048).4.The change of biochemical indexes(ALT,AST,TP,ALP)before and after transfusion of cryopreserved platelets had no statistical difference in group 3,the changes of biochemical indexes in group 1 and 3infusion platelets were not statistically significant(P<0.05).No significant changes were found in the physical examination before and after infusion of frozen platelets in group 3 patients,and no toxic and side reactions related to DMSO occurred.Group 3 patients were classified by disease.there was no significant difference in the change of biochemical indexes between different infusion doses of the same disease(P<0.05).5.The biochemical parameters of the 50 patients were not statistically significant(P<0.05),and no abnormalities were found in the physical examination,and no adverse reactions related to DMSO were found.Conclusion:1.Cryopreserved platelets can effectively improve the platelet count in patients,but the fresh platelets did better,the effective rate of infusion is equivalent to that of concentrated platelet,apheresis platelet.2.The adverse transfusion reaction cryopreserved platelets is lower than of manual concentrated platelets 3.Dimethyl sulfoxide the preservation liquid of cryopreserved platelets has no obvious toxic side effect on human body,and no significant effect on ophthalmology,nervous system and Clinical examination.So,washing is not needed when we use cryopreserved platelets in clinical.4.The preservation time of cryopreserved platelets keep long,and the risk of homoimmune reaction and virus transmitting is small.Cryopreserved platelets has great advantages in terms of reservation and efficiency.To explore the approach of prolonging the shelf time of platelets would do much favor to the development of transfusion medicine,and has important practical and guiding significance.
