Risk Factors On The Short-term Outcome Of DCD Kidney Transplanted Recipients:A Single-center Study

Objectives: To discuss donors` and recipients` risk factors for the short-term outcomes of kidney transplanted recipients from Donation after citizen`s deceased(DCD).Methods: We retrospectively analyzed the informations of donors and recipients at our Kidney Transplantation Center from January 1,2015 to Feb 28,2017.We compared the risk factors of delayed graft function(DGF),pneumonia and one-year estimated-Glomerular filtration rate(e GFR)of recipients.Results: The rates of DGF and pneumonia in kidney transplanted recipients from DCD were 28.3% and 27.2%,respectively.The average value of one-year e GFR was78.10±22.32ml/min/1.73m2.On univariate analysis,we found that cerebral hemorrhage as the cause of brain death(P=0.022),the history of Cardio-pulmonary resuscitation(CPR)(P=0.033),terminal systolic blood pressure below 100 mm Hg(P=0.022),and pretransplantation dialysis time beyond one year(P=0.012)were risk factors for DGF.Moreover,donors’ age beyond 50(P=0.030)and male recipients(P=0.003)were risk factors for pneumonia.When donors’ age were beyond 50,one-year e GFR of the recipients were lower(P=0.008).However,female recipients(P=0.005)or recipients who underwent peritoneal dialysis before transplant(P=0.028)had higher one-year e GFR.On multivariate analysis,cerebral hemorrhage as the cause of brain death(P=0.011),the history of Cardio-pulmonary resuscitation(CPR)(P=0.020)and terminal systolic blood pressure beyond 100 mm Hg(P=0.034)were independent predictive factors for DGF.Donors’ age beyond 50(P=0.049)and female recipients(P=0.010)were showed significant differences in the incidence of pneumonia.In multiple linear regression model,donors’ age beyond 50 had negative correlation with one-year e GFR(P<0.001),while female recipients(P=0.010)and recipients who underwent peritoneal dialysis before transplant(P=0.001)had positive correlation.Conclusion: Cerebral hemorrhage as the cause of brain death,the history of CPR and terminal systolic blood pressure beyond 100 mm Hg were independent predictive factors for DGF.The age of donors beyond 50 and gender of recipients were risk factors for pneumonia.Donors’ age beyond 50 had negative correlation with one-year e GFR,while female recipients and recipients who underwent peritoneal dialysis before transplant had positive correlation.These factors should be considered while evaluating potential donors.
