| Objective To investigate the correlation between pulmonary edema area and extravascular lung water index(EVLWI)in patients with acute left heart failure,according to pulmonary edema area initially determine the extent of pulmonary edema in patients with acute left heart failure,and guide the liquid management.Methods 60 patients with acute left heart failure were included,Simultaneous use of PICCO monitor and X-ray at the bedside,Access to EVLWI,pulmonary edema area,after 3 days of treatment,according to the improvement of the patient’s symptoms and the change of heart function grading,the patients were divided into improvement group and worsening group.EVLWI and pulmonary edema area were detected again in the two groups.The changes of EVLWI and pulmonary edema area before and after treatment were analyzed.Further analyze the correlation between the above two indicators.Results Analyze the EVLWI,before and after treatment in the improved group were:14.59±4.71,8.5±2.02;Before and after treatment in the worsening group were:14.94±5.57,20.1±5.57.Improved group p<0.001,the difference was statistically significant,EVLWI decreased after treatment in the improved group.Worsening group p=0.044,the difference was statistically significant,EVLWI increased after treatment in the worsening group.Analyze the pulmonary edema area,before and after treatment in the improved group were: 0.64±0.23,0.14±0.08;Before and after treatment in the worsening group were: 0.58±0.19,0.92±0.08.Improved group p<0.001,the difference was statistically significant,pulmonary edema area decreased after treatment in the improved group.Worsening group p<0.001,the difference was statistically significant,pulmonary edema area increased after treatment in the worsening group.The above results show that the changes in EVLWI and pulmonary edema area before and after treatment in the two groups are similar,research the correlation between the two factors find that: The pulmonary edema area was positively correlated with EVLWI(r=0.405,P<0.001),there is a linear regression between the two variables,regression equation is: EVLWI=11.007+pulmonary edema area*3.260.EVLWI was positively correlated with BNP(r=0.598,p<0.001),while negatively correlated with left ventricular ejection fraction(LVEF)、cardiac index(CI),were(r=-0.266,p=0.04),(r=-0.284,p=0.028).EVLWI suitable as a predictor of acute left heart failure,the results calculated cut-off value of 10.9,a sensitivity of 85.3%,a specificity of 85.7%;PVPI is suitable as a predictor of acute left heart failure with a cutoff of 1.95,a sensitivity of 84.9% and a specificity of 42.9%.Conclusion Pulmonary edema area was significantly correlated with EVLWI in patients with acute lefe heart failure,predict extravascular lung water index by pulmonary edema area and determine the severity of pulmonary edema,to guide the treatment. |
