Objective:To evaluate the effects of Pipeline Embolization Device(PED)in the treatment of intracranial wide-necked aneurysms in a short-term follow-up period.Methods:From October 2015 to September 2016,15 patients with unruptured intracranial wide-neckd aneurysm received Pipeline Embolization Device treatment at the Department of Neurosurgery of the Second Affiliated Hospital of Nanchang University,the clinical and imaging record were analyzed.The occlusion rate and patency status of the parent artery were evaluated according to the Kamran Scale,the clinical outcomes were assessed by the modified Rankin scale(mRS).All cases were followed up with telephone call or outpatient(within 3months after treatment)each month.The Friedman test was used to compare the difference between before-treatment,after-treatment and follow-up。Results:In this study,a total of 15 aneurysms were unruptured wide-necked aneurysm,15 PED were implanted,including 13 carotid ophthalmic aneurysms,1 posterior communicating artery aneurysms,1 cavernous sinus aneurysms,aneurysm diameter ranged from 3.5~16.2 mm.All patients were treated with PED,and each patient was implanted with 1 device.contrast-retention were obviously observed in aneurysm as stent implanted,aneurysm was thinner after treatment,the technical success rate was100%.All 15 cases were grade 2 on aneurysm,grade a on parent artery(grad 2a).2cases(>10 mm)aneurysm were staged coil embolization,with no neurological deficit performance during perioperative.Postoperative 6 to 12 months of cerebral angiography review,14 cases were grade 4 on aneurysm,grade a on parent artery(grad 4a);1 case was grade 3 on aneurysm,grade a on parent artery(grad 3a),which was followed up in 6 months and 12 months after the surgery,Aneurysm volume residual both are existed.No branch artery occlusion and neurological deficit performance were found,Modified Rankin Scale are grade 0 in all 15 cases.Friedman test showed that there were statistically significant differences between before-treatment,after-treatment and follow-up(X~2=30,df=2,P<0.05).Conclusions:Short-term follow-up studies have shown that the use of Pipeline Embolization Device for the treatment of intracranial unruptured wide-necked aneurysms is safe and effective,with a high occlusion rate,and aneurysm embolization is stable and effective,nonuse or a small amount of coil embolisation treatment is also effective,the long-term effect needs further follow-up。...
