Survey On The Variation Trend Of Hospitalization Expense Of Three Common Diseases In A Military Hospital

Objective: The aim of current study was to discuss the variation trend of hospitalization expense of three common diseases in a military hospital,therefore providing scientific reference for controlling hospitalization expenses.Methods: Through the Hospital Information System(HIS),we collected the Patient information of hyperthyroidism,type ? diabetes and lumbar intervertebral disc hernia from January 1,2011 to December 31,2015 and analyzed the hospitalization expenses and change trend.Analytical methods included descriptivestatistics,non-Parametric test and correlationanalysis.Results:(1)The number of lumbar intervertebral disc herniain patient has increased from 2011 to 2015.The percentage of males of type ? diabetes and lumbar intervertebral disc hernia were greater.The gender difference of hyperthyroidism in each year was statistically significant(P<0.05).The average hospitalization days went down(except the hyperthyroidism)and the differences were statistically significant(P>0.05).The surgery rates declined(except the cataract)and the differences(except the type ? diabetes)were statistically significant(P<0.05).(2)The average hospitalization expenditures per time of appendicitis increased from 3415.55 yuan to 5322.69 yuan between 2011 and 2015 and the growth rate per annum was 11.73%.The average hospitalization expenditures per time of type ? diabetes declined from4122.56 yuan to 4080.16 yuan between 2011 and 2015 and the growth rate per annum was-0.26%.The average hospitalization expenditures per time of lumbar intervertebral disc hernia were declined from13121.77 yuan to8742.41 yuan between 2011 and 2015 and the growth rate per annum was-9.65%.The differences of 3 diseases' average hospitalization expenditures per time in each year were statistically significant(P<0.05).(3)The average hospitalization expenditures per day of hyperthyroidism increased from 1744.24 yuan to 2635.74 yuan between 2011 and 2015 and the growth rate per annum was 10.87%.The average hospitalization expenditures per day of type ? diabetes declined from 410.63 yuan to402.09 yuan between 2011 and 2015 and the growth rate per annum was-0.52%.The average hospitalization expenditures per day of lumbar intervertebral disc hernia declined from 970.44 yuan to757.91 yuan between 2011 and 2015.(4)Consultation costs,inspection costs,and drug costs took a high proportion in the cost structure of hyperthyroidism.Drug costs,consultation costs,and inspection costs took a high proportion in the cost structure of type ? diabetes.Surgery costs,consultation costs and inspection costs took a high proportion in the cost structure of lumbar intervertebral disc hernia.(5)The influencefactors of hyperthyroidism'hospitalization expenditures were year,examination cost ratio,pattern of payment,hospital stay,drug cost ratio and age.The influencefactors of type ? diabetes' s hospitalization expenditures were year,examination cost ratio,hospital stay,pattern of payment and year.The influencefactors of lumbar intervertebral disc hernia's hospitalization expenditures were examination cost ratio,hospital stay,drug cost ratio and age.Conclusion:(1)The number of inpatient and the average hospitalization days were on the rise(except the lumbar intervertebral disc hernia)and the average hospital stay declined(except the hyperthyroidism).(2)The average hospitalization expense of Hyperthyroidism was on the rise and the other two diseases' went down.(3)The average hospitalization expenditures per day of hyperthyroidism was on the rise and the other two's went down.(4)The consultation costs,examination cost and drug cost took higher proportion.(5)The hospital stay,patternofpayment,drug cost ratio and examination cost ratio had great influence on the hospitalization expenditures.Suggestion:(1)Keep a moderate scale of development of military hospitals.(2)Control the health costs by the reform of Joint Reformation for public health services,medical insurance and medical production-circulation.(3)Highlight the subject status of the medical personnel.(4)The clinical pathway should play an important role in the control of health costs.
