Objective To investigate the relationship between post-stroke depression(PSD)and cognitive impairment.Methods To collect 102 patients with PSD admitted to the Department of Neurology,Ningxia People’s Hospital from October 2015 to October 2017 as study group(PSD group),122 patients diagnosed with stroke but not with PSD during the same period served as controls(non-PSD group).Study general patient data,past history,NIHSS scores,laboratory indicators,imaging studies.In the PSD group and non-PSD group,the scores of cognitive function were scored on the 14d±2d and 3 months after stroke and MMSE and MoCA.14d±2d after stroke in the PSD group using HDRS scale to assess the degree of depression.The HDRS scale was used to assess the degree of depression in the PSD group 14d±2d after stroke.The scores were divided into three groups: mild PSD,moderate PSD,and severe PSD.The non-parametric rank sum test was used to compare the differences in MMSE scores between the three groups.The MMSE scores of the light,middle and heavy groups were further compared by the rank sum test.The corrected test level was α’=0.05/3=0.017.Severity of PSD(HDRS score)and severity of cognitive impairment(MMSE score)were compared using Spearman correlation test.Results(1)At different time points after stroke,the scores of MMSE and MoCA in the PSD group were lower than those in the non-PSD group.The difference was statistically significant(P<0.05).(2)on the 14d±2d and 3 months,PSD group scores in visual space/execution ability,attention,memory and orientation were lower in all subjects than in non-PSD group.The difference was statistically significant(P<0.05).(3)The overall difference of MMSE scores among the three groups of PSD was statistically significant.The MMSE scores among the light,medium,and heavy groups were further compared with non-parametric extended t-test.The corrected test level was α’=0.05/3=0.017.The mean MMSE score in the moderate PSD group was lower than that in the mild PSD group,but the P = 0.089>0.017,suggesting that the difference was not statistically significant;the mean MMSE score of the severe PSD group was significantly less than the mild PSD group,and P=0.010<0.017,indicating that the difference was statistically significant.(4)The Spearman correlation test between HDRS score and MMSE score in PSD group showed that R(correlation coefficient)=-0.372,P=0.008<0.01,with statistical significance.Conclusion(1)PSD can aggravate cognitive impairment and affect the improvement of cognitive function.(2)PSD may have greater damage to the cognitive space of the patient’s apparent space and executive function,attention,memory,and orientation.(3)The more serious the PSD is,the more severe the cognitive impairment is.
