Objective By comparing the levels of golgi protein 73(GP73)in serum and liver tissue of patients with chronic hepatitis B(CHB),along with the FS value of liver fibrosis,compared with the biopsy of liver tissue.,to investigate the diagnostic value of GP73 alone or in combination with Fibroscan in the early stage of hepatic fibrosis(HF)in CHB patients.Methods 1.222 patients were collected from December 2014 to December 2017 in the Department of Infectious Diseases of the Ningxia Medical University General Hospital,including 68 patients with mild CHB,41 patients with moderate,51 patients with severe,and 62 patients with cirrhosis;while 30 healthy patients were selected at the same time for the control group;2.Enzyme-linked immunosorbent assay(ELISA)was used to detect serum GP73 levels;115 cases of CHB patients underwent hepatic histological examination,and immunohistochemistry was used to detect GP73 expression in liver tissue.GP73 expression in liver tissue was evaluated by two Pathologists which were blinded to the study.Routine staining of liver tissue was used to determine the degree of inflammation(G)and fibrosis(S);while liver stiffness values were measured using Fibroscan;3.Using SPSS 22.0 statistical software to analyze the correlation between serum and liver GP73 levels,and its combination with Fibroscan in the diagnosis of early HF of CHB patients.Results 1.Serum GP73 in healthy controls,CHB patients with mild to severe three groups,cirrhosis group were compared between groups,the difference was statistically significant(p <0.01),serum GP73 in CHB moderate severe and cirrhosis patients were higher Compared with mild CHB group and healthy control group(p<0.01),there was no significant difference in serum GP73 between cirrhosis group and severe group,mild group and healthy control group(p>0.05);2.There were significant differences in serum GP73 levels between different HF stages in patients with CHB(p<0.01).2.In the 115 CHB patients undergoing liver biopsy,expression of GP73 in negatives,weakly positives,moderately positives and strongly positives in liver tissues were 24 cases,17 cases,47 cases,and 27 cases respectively.With the GP73 level expression in the liver tissue being negative,weakly positive,moderately positive,and strongly positive,the corresponding serum GP73 levels had significant differences between groups(p<0.01).There was a significant difference between liver tissue and serum GP73 expression in CHB patients(r=0.393,p<0.01).3.The GP73 levels in liver tissue and serum were positively correlated with Fibroscan values(r=0.361,0.556,respectively,p < 0.01),GP73 in Peripheral blood and liver tissue were positively correlated with HF stage(S),respectively(r values were 0.364,0.216,p<0.05).4.The ROC curve for the expression of GP73 and FS in peripheral blood of CHB patients in liver fibrosis stage(S?2)demonstrates area under the curve of 0.68,0.70,respectively(sensitivity of 54.5%,52.3% and specificity 78%,72%),and the p value was <0.05.The area under the combined ROC curve of the two is 0.7.Conclusions 1.Serum levels of GP73 in CHB patients were higher than those in healthy subjects.With the development of hepatic fibrosis and liver cirrhosis,GP73 showed a significantly increasing trend,indicating that GP73 may be used as a serum marker for liver fibrosis progression;2.Serum GP73 alone and in combination with Fibroscan have high specificity and sensitivity for assessing the degree of hepatic fibrosis and may have a certain role in the diagnosis of hepatic fibrosis.
