The ability to regulate emotions is an important part of adapting to society,while the emtion prossing and regulation deficits are at the core of bipolar disorder(BD)and major depression disorder(MDD).In the past decade,much work has focused on the clinical diagnosis and treatment of the diseases,wheres little attention to the social cognitive aspects(especially the ability of emotional regulation)of patients with BD and MDD.A large number of studies have shown that reappraisal is an effective means of regulating emotions,and is widely used in daily life.It is necessary to explore the differences between patients with BD and MDD.At first,the present study would discus the use of cognitive reappraisal and expression inhibition in BD and MDD.The aim of this study was to further investigate potential differences of cognitive reappraisal between BD and MDD,including the subjective assessment and NIRS outcome.Which may help to explore whether it could be usefull for patients,and may also help to provide the basis for the differential diagnosis of both bipolar depressive disorder and unipolar depression.The emotional regulation scale(ERS),which is divided into two dimensions,was used to investigate the cognitive reappraisal and expression inhibition of 19 patients with bipolar depressive disorder,25 patients with unipolar depression and 22 controls.In the ERS,the scores of expression inhibition had no statistical differences between patients and control group,but the use of cognitive reappraisal was significant less than the control group.Which means patients with depressive less using cognitive reappraisal strategy than the normal population.In the cognitive reappraisal 3󫢪 test,with valence(neutral,negative or positive)and instruction(look or reappraisal)as within-subject factors,compare the different groups about subjective emotional experience and brain activation.The subjective scores of the negative emotion stimuli were significantly higher than those in natural films,and the scores of the patient groups were significantly higher than those before the cognitive reappraisal.The subjective scores before and after reappraisal were not significant but significantly lower than the control group.It means the cognitive reappraisal strategy was usefull to regulate emotions,especially the effective regulation of negative emotions,for BD patients also have a good emotional conditioning ability and can effectively improve the subjects subjective emotional experience.The results of NIRS analysis showed that the brain activity of channel 6,14 and 16 changed significantly in the MDD group,and the activities of the patient groups were significantly different on the channel 16.There was a significant change in the response to different emotional types before and after cognitive revaluation on the channel 13,17,19.That is,cognitive reappraisal of the activation of the prefrontal cortex in the central and right,single-phase depression in patients with both sides of the performance of DLPFC and the right side of the PFC activation is relatively weakened,BD group on the right side of the PFC relative to the control group activated Weakened,MDD group in the cognitive reappraisal of the right side of the activation of DLPFC than BD group weakened.
