| ObjectiveThis research is based on preliminary study,we found that patient with gastro-esophageal reflux disease(GERD)of the dorsal segment of the Governor Vessel exists obviously regular tenderness pressing pain,adopting the therapeutic method of acupuncture the T3-T12 section of the spine below the spinous process(including non-acupionts on the vessel),meanwhile comparing the clinical practice with the patients who taking Omeprazole capsules of western medicine group.Mainly observing the improvement of physiology and psychological symptom,then further objective evaluate the acupuncture treatment of dorsal segment of the Governor Vessel of the clinical curative effect of GERD,and observing the GERD patients changes of tenderness pressing pain acupuncture this section(T3-T12)and its clinical significance.MethodFrom January 2015 to June 2016,60 cases of gastro-esophageal reflux disease were divided into acupuncture group and medication group,30 cases in each group.All the cases are consistent with diagnostic criteria of GERD and sign informed consent.Minimum age 26 years old,biggest 76 years old;The shortest duration 2 months,the longest 20 years in the acupuncture group.Minimum age 25 years old,biggest 68 years old;The shortest duration 3 months,the longest 15 years in the medication group.Two groups of patients in basic information(sex,ageand course of the disease),there is no difference between two groups(p>0.05),possessing comparability.For patients of the acupuncture group,we take 0.25mm*25mm disposable sterility acupuncture pin(Beijing Zhongyan Taihe Medical Instrument Company),selecting the depression spots below the spinous processes from T3-T12,including both acupoints[Shenzhu(GV12),Shendao(GV11),Lingtai(GV10),Zhiyang(GV9),Jinsuo(GV8),Zhongshu(GV7),Jizhong(GV6)]and non-acupoints(T4,T8,T12),the pinpoint stab upward about 15mm,subject to the needle arrival of qi,mild reinforcing and attenuating,hand-manipulating of needle every 15 minutes,retaining needle for 30 minutes,treating every Tuesday,Thursday and Saturday in the morning for 8 weeks.Patients of the medication group were asked to take Omeprazole capsules(20mg/time)twice a day for 8 weeks.The curative effects were assessed by "Reflux Disease Questionnaire" score(RDQ score of heartburn,retrosternal pain,acid regurgitation and food regurgitation,0-40 points),"Self-Rating Anxiety Scale"(SAS)and "Self-rating depression scale"(SDS),comparison and access the tenderness degree and threshold of T3-T12.Results1.Score comparison on RDQ before and after treatment in both groups:The patients in acupuncture and western medication group do not have statistical significance before treatment on the scores of RDQ(p>0.05);After the treatment,the scores of RDQ is obviously decreased in the patients of acupuncture and medication group(P<0.01),acupuncture group was significantly superior to the medication in lowering RDQ(P<0.01).2.Compare two groups of patients with RDQ scale effect after treatment:There are 8 cases in curable,12 cases had marked effect,8 cases were in progress,2 cases were invalid,0 case was worsened in the acupuncture.There are 5 cases in curable,7 cases had marked effect,9 cases were in progress,6 cases were invalid,3 cases were worsened in the western medication group.After statistical analysis found that the markedly effective rate of two groups are 66.7%and 40%,showed significant differences(P<0.01),acupuncture curative effect is better than western medicine group(P<0.05).3.Score comparison on the simultaneous symptoms of GERD before and after treatment in both groups:The patients in acupuncture and western medication group do not have statistical significance before treatment on the scores of simultaneous symptoms of GERD(p>0.05);fter the treatment,the scores of simultaneous symptoms of GERD is obviously decreased in the patients of acupuncture and medication group(P<0.01),acupuncture group was significantly superior to the medication in lowering simultaneous symptoms of GERD(P<0.05).4.Compare two groups of patients with the simultaneous symptoms of GERD scale effect after treatment:There are 3 cases in curable,17 cases had marked effect,8 cases were in effective,2 cases were ineffective in the acupuncture.There are 1 cases in curable,6 cases had marked effect,14 cases were in effective,9 cases were ineffective in the western medication group.After statistical analysis found that the total effective rate of two groups are 9%and 70%,showed significant differences(P<0.05),acupuncture curative effect is better than western medicine group(P<0.05).5.Compare two groups of patients with the change of tenderness degree:the tenderness degree of two groups of patients were after quantization,then through statistics processing,two groups after treatment in the T3-T12 section of the spinous process tenderness decreased than before(P<0.05);comparison between groups,the acupuncture group found tenderness relief degree was significantly better than the western medicine group(P<0.05).6.Compare two groups of patients with the change of pain threshold:Two groups of patients with pain threshold changes after treatment in the T5-T12 section of the spinous were significantly increased than before(P<0.05);comparison between groups,at T5-T12 spinous process tenderness than western medicine group(P<0.05),and the high threshold in the acupuncture group T5-T7 spinous process under the pressure pain threshold improved significantly more than the western medicine group(P<0.01),there is no obviously change on T3,T4 section of the spinous before and after tenderness in the spinous process of under the treatment for two groups(P>0.05).7.Comparison of two groups of patients SAS,SDS score:acupuncture group and Western medicine group SAS,SDS score before treatment were not statistically significant(P>0.05),comparable;after treatment,two groups of patients with SAS,SDS score significantly decreased,the difference was statistically significant(P<0.05);and acupuncture group than in the western medicine group were significantly decreased(P<0.05).Conclusion:1.the Du Meridian dorsal segment T3-T12 segment spinous process to improve the GERD patients with reflux symptoms significantly better than western medicine group,acupuncture as a simple low-cost means of operation,it has the characteristics of integrity and mutual,it through the whole multi-link,multi-level and multi-target conditioning and individualized treatment for patients with satisfactory therapeutic effect,clinical and lasting effect,low recurrence rate,worthy of patient trust and application.2.the Du Meridian dorsal segment T3-T12 segment spinous process can improve the GERD patients with back pain threshold,so it can be take the section of the Du Meridian as a preliminary diagnostic criteria for GERD and acupuncture effect index.It is of great significance to the prevention and treatment of GERD,and to provide clinical evidence for early detection of disease and effective treatment.3.the Du Meridian dorsal segment T3-T12 segment spinous process and improve the mentality of anxiety and depression in GERD patients,and the curative effect is better than western medicine group,provide new clinical thinking for the treatment and the diagnosis for somatopsychic illness,this is an important manifestation of TCM theory for unity of body and soul. |
