BackgroundScience of seasonal febrile disease is a kind of acute exogenous febrile disease caused by pathogenic warm,with fever as the primary symptom and body's systemic reaction to pathogenic warm,which can be seen in various stages of the development of science of seasonal febrile disease.Exogenous fever occurs in lungs at first,and is closely related with gastrointestinal tract.Yangming plays an important role in the degree and prognosis of fever.Therefore,Yinlai decoction is an empirical prescription to treat fever in the qifen and weifen,with principles of treating lung and stomach,removing pathogens up and down at the same time,clearing the lung and harmonizing the stomach,and reconciling the outside and inside.Good effect has been obtained in clinical application,but its related antipyretic mechanism is not yet clear and remains to be further clarified.Based on the theory of"set-point(SP)" in modern pathophysiology,it is considered that fever is caused by endogenous pyrogen in elevating the thermoregulatory set point and causing a regulated rise in body temperature.Therefore,the studies on specific antipyretic mechanism of traditional Chinese medicine(TCM)is mainly focused on the "SP" theory,including the regulation of central thermoregulation medium,the regulation of endogenous pyrogen,the regulation of discharge frequency of the temperature-sensitive neurons and block of the conduction of Toll like signaling pathways.However,TCM compound may work through the joint intervention of complex multipath,and its related antipyretic mechanism remains to be further studied.The brain gut axis is a bidirectional regulation axis of the central nervous system and gastrointestinal function,and the neuro-endocrine-immune network is an important connection between the brain and intestine.The brain gut peptides,which are secreted and released by the brain-gut neuroendocrine cells,are important targets.Because some targets of brain gut axis,mainly brain gut peptides,are closely related to the incidence and prognosis of fever,it is of great value and clinical significance to explore the mechanism of TCM compound intervention to reduce the level of brain gut peptide.In summary,this research explores the yeast related antifebrile mechanism of Yinlai decoction through the animal experiment method,expounds the way and targets to realize the antipyretic effect of Yinlai Decoction,searches for the possible relationship between the brain gut peptide and inflammatory cytokines,and provides a modern research basis for the application of simultaneous treatment of lung and stomach.This study was divided into two parts:literature review and experimental research.Literature review were made of two parts:the studies on mechanism of antipyretic effect of TCM were reviewed and summarized in the past ten years,from the perspective of modem experimental research and the etiology,pathogenesis,syndrome differentiation and treatment of fever in the qifen and weifen were reviewed and summarized,from the perspective of TCM theory.Experimental research:PurposeTo make clear the antipyretic effect of Yinlai Decoction by this study,and exploring the antipyretic mechanism of Yinlai Decoction.We will state the relevant pathway and targets in the antipyretic process of Yinlai Decoction,and discus the relationship between brain gut peptide and inflammatory factor,then obtain some experimental evidence of the treatment of the principles of treating lung and stomach.MethodWe choose the method of animal experiment and make the fever rat model by subcutaneous injecting yeast.Then observing the antipyretic effect of Yinlai Decoction through testing serum IL-1?,IL-6,TNF-? and cAMP,5-HT,ghrelin in hypothalamus by ELISA and RIA.ResultsThere was no significant difference in the basal body temperature of rats in each group(P>0.05);5h after modeling,compared with normal group,there was a significant difference on temperature between model group,each dose of yinlai decoction group,and aspirin group(P<0.01);3h after the last administration,compared with model group,each dose of yinlai decoction group and aspirin group had a significant difference on temperature(P<0.01).Compared with model group,there was a significant difference on the level of IL-1?,IL-6,TNF-? and cAMP between each dose of yinlai decoction group(P<0.05),whose mean values lower than the model group.Compared with model group,there was a significant difference on the level of 5-HT between each dose of yinlai decoction group(P<0.05),whose mean values lower than the model group;Compared with model group,there was a significant difference on the level of ghrelin between each dose of yinlai decoction group(P<0.05),whose mean values higher than the model group.ConclusionsYinlai decoction has antipyretic effect on yeast induced fever rats,and the antipyretic effect of middle dose group is the best.Its antipyretic effect reflects on the curve in lowering the body temperature peak and pushing forward the body temperature peak platform,thereby reducing the thermal potential and shortening the duration of fever.Yinlai decoction may decrease the rat serum endogenous pyrogen levels through direct interaction with IL-1?,IL-6,TNF-? and cAMP,increase hypothalamic cAMP level,which makes the temperature set point down-regulated,and realize the antipyretic effect.Yinlai decoction's antipyretic effect can also be related to decreasing hypothalamic 5-HT level and increasing hypothalamic ghrelin level.To sum up,as a TCM compound,Yinlai decoction may achieve the antipyretic effect through regulating complex multipath,and the role of each pathway can be either an independent parallel relationship or an interactive relationship.Yinlai decoction can achieve the antipyretic effect not only through decreasing the enogenous pyrogen level and actively regulating factors level,but also be related to regulating the relevant brain-gut peptide.
