Clinical Observation Of 60 Cases Of Calcaneal Fracture Treated With Chi's Orthopedics Combined With Kirschner Wire Percutaneous Internal Fixation

Purpose: Clinical curative effect of calcaneal fractures was studied in the retrospective analysis of open reduction and internal fixation in calcaneal fracture with percutaneous reduction fixation screws.From the research according to the anatomy,injury mechanism,pathological changes,diagnosis combined treatment of calcaneal fracture and the prevention of complications and other theories,analysis of clinical efficacy of two kinds between internal fixation methods in the treatment of type ? and Sander ? type calcaneal fractures was compared and guided the treatment of calcaneal fracture analysis from the advantages and disadvantages in order to sum up experience,enhanced the curative effect,improved the quality of life of patients,reduced the complications and provided new therapeutic options in the clinical treatment.Materials and methods: All cases were from Fushun Hospital of traditional Chinese medicine bone ward,the patient were suffered from admission of calcaneal fractures during 2015.1-2016.12.According to the diagnostic criteria,inclusion criteria and exclusion criteria,60 cases of Sanders fracture patients with type ? and Sanders type ? calcaneus fractures were selected,in which of the cases that 42 of male,18 of female whose age arranged from 22 to 75 years old with an average age of 43.7 years old,the causes of injury was made via 18 cases in walking fall down,14 cases in traffic accident,28 cases in falling from high position.They were randomly divided into two groups as treatment group and control group,each group had 30 cases.The treatment group was treated with bonesetting percutaneous reduction combine percutaneous reduction and internal fixation with cannulated screws;control group were treated with open reduction and plate screw fixation.After the operation,the two groups of patients were given traditional Chinese medicine(three phases)of Fushun Hospital in oral administration together with functional exercise.Follow-up research were performed from 1 to 6 months after the operation,two groups were recorded with hospitalization time,cost of hospitalization,complications,relevant anatomy reset(B?hler,Gissanc,two groups of foot score index and calcaneal height)before and after 1 days together with one year after theoperation.The evaluation data for statistical software was SPSS16.0 in analyzing and processing.There was no significant difference between the two groups in gender,age,preoperative and postoperative imaging of the four indicators.Results: Two methods in treatment of Sander type ? and type ? calcaneal fractures can provide treatment effect.In the score of evaluting function,the treatment group and the control group with P > 0.05 and the study in anatomical imaging,treatment group and control group had no significant difference(P > 0.05);however,compared with the control group,the treatment group hospitalization time,intraoperative blood loss,treatment cost and ostoperative complications have clear advantages with P < 0.01.Conclusion:The clinical efficacy of the two methods in the treatment of type ? Sander and type ? calcaneal fractures is quite effective.However,compared with the open reduction and plate screw fixation,poking reduction and internal fixation with hollow screws with relatively simple operation,has more advantages in saving operation time,less bleeding and the economic burden is relatively low,also,quick healing,less postoperative complications are included;at the same time,surgical trauma in small operation could relatively ease the pain of the patients and easier for the patients to accept.
