Objective: To compare the efficacy of percutaneous nephrolithotomy(PCNL)and minimally invasive percutaneous nephro Lithotomy(mPCNL)and percutaneous nephrolithotomy with the all-seeing needle system(Microperc)for the treatment of upper urinary tract calculi.Methods: Collection of 100 cases of upper urinary calculi from 09 months to 04 months from 2015 to 2018 by percutaneous nephroscope,standard channel percutaneous nephroscope and visual percutaneous nephroscope in the period of visual percutaneous nephroscope.Among them,36 cases were treated by PCNL(group A),34 cases were treated with mPCNL(group B),and 30 cases were treated with Microperc(C group).In group A,there were 36 cases of renal calculi with an average diameter of 2.7 cm,and 34 cases of renal calculi in group B and 2.4 cm in diameter;30 cases in group C and 2.2 cm in diameter.Recorded and compared with three groups of patients with operation time,a stone clearance rate,postoperative recovery time of intestinal ventilation,first ambulation time,postoperative hemoglobin decrease,postoperative HCT decreased,blood transfusion rate,postoperative fever,infection,postoperative analgesic requirement rate,hospitalization time.Result:Group A operation time was(94.47 + 6.80)min renal calculi clearance rate was 83.9%(30 / 36),postoperative intestinal ventilation recovery time(16.42 + 2.53h), first ambulation time(44.61 + 5.30h),postoperative Hb value was decreased(7.44 + 3.12 g g/L)and after operation the decrease of HCT(3.97% + 1.66%),postoperative blood transfusion rate(2/36),infection rate after operation was 5.6%(2/36),the postoperative analgesic requirement rate is 10/36,the hospitalization time was 7.64 + 1.27d;The operation time of B group is(115.50 + 6.37)min renal calculi clearance rate was 80.2%(27 / 34),postoperative intestinal ventilation recovery time(16.06 + 1.49 h h),first ambulation time was 42.71 + 3.86h),postoperative Hb value was decreased(7.68 + 2.43g/L),postoperative HCT decreased the value of(3.88% + 1.80%),postoperative blood transfusion rate(1/34),infection rate after operation was17.6%(6/34),the postoperative analgesic requirement rate is 5/34,the hospitalization time was 5.97 + 1.30d;The operation time of C group is(101.40 + 7.17),renal calculi clearance rate was 81%(24 / 30),postoperative intestinal ventilation recovery time(16.03 + 2.01h),first ambulation time(39.83 + 4.59h),postoperative Hb value was decreased(7.33 + 2.01g/L),postoperative decrease of HCT(3.70% + 1.32%),postoperative blood transfusion rate(1/30),infection rate after operation was 16.7%(5/30),the postoperative analgesic demand rate is 2/30,the hospitalization time was 5.30 + 1.86d;Conclusion: PCNL,mPCNL and Microperc had no significant difference in surgical clearance rate.In the operation time,the time for the comparison of the three standard PCNL was lower than that of mPCNL,and the Microperc was lower than that of the mPCNL.three groups,and there was no significant difference in the blood transfusion rate,the postoperative gastrointestinal ventilation time,the hemoglobin drop value,and the HCT changes.The infection rate of fever after standard PCNL was significantly less than that of mPCNL and Microperc,and there was no significant difference between the latter two.Microperc and mPCNL were better than standard PCNL in the time of first ambulation,hospitalization time and postoperative pain,and there was no significant difference between Microperc and mPCNL. |
