Background/ObjectivesPassive smoking during pregnancy is one of the risk factors that affect pregnancy outcomes. Therefore, reducing second-hand smoke in the environment is more prominent.Although there have been some researches on the passive smoking during pregnancy, there are few studies on the passive smoking status of pregnant women and the outcome of pregnancy in Shandong province. This study mainly aims to understand the knowledge behavior attitude of pregnant women about passive smoking, analyze the relationship between passive smoking and pregnancy complications and pregnancy outcome. In order to develop related tobacco control intervention measures, reduce the occurrence of adverse pregnancy outcome and promote fetal health.MethodsIn this study,we investigated the pregnant and pregnant women in the obstetrics department of the Qilu hospital of Jinan City,Dezhou City hospital, and the maternity and child care hospital of Dezhou city since July 2017 to December 2017.The basic information of pregnant women was collected through questionnaire survey, including the general personal characteristic, smoke exposure situation, and knowledge attitude and behavior of passive smoking. The pregnancy outcome was recorded from the patient's medical records after delivery.SPSS 13.0 software was used to organize and analyze the data.T-test and variance analysis were used for the analysis of metering data, and chi-square test was adopted for metering data. The correlation between passive smoking in pregnancy and pregnancy outcome was discussed in detail by using the multivariate Logistic regression.P<0.05 indicated that the difference was statistically significant.ResultsA total of 1,027 valid questionnaires were obtained and the effective response rate was high(98.4%).The age of pregnant women was mainly 25 to 29 years old(35.4%),and the passive smoking rate during pregnancy was 23.6%.Passive smoking was associated with the site of investigation, living environment, whether there is a factory nearby, childbearing history, history of miscarriage, mate drinking frequency, mate personality, spouse smoking history, spouse smoking during pregnancy, passive smoking before pregnancy, the guest smoking in the home, the home is affected by the non-smoking smoke, smoking places at home. The differences of passive smoking group and the control group in the understanding of passive smoking were statistically significant(P<0.05).There were 36.5%,17.4%,17.4%,and 15.1%of pregnant women takeing measures to avoid or prevent husband, family members, guest, and colleagues to stop from smoking. The total number of pregnant women with pregnancy complications was 470.47 Of them, pregnant women ended up with abortion or induced labor. And 375 pregnant women gave birth successfully, and the delivery method was mainly cesarean section(61.1%).The birth information of 357 newborns was obtained by excluding twin pregnancy(n=18).The incidence of pregnancy complications was 54.3%,the incidence of exposure group was 48.8%,and the incidence of control group was 64.1%.The incidence of adverse pregnancy outcome was 42.7%,the incidence of exposure group was 50.4%,and the incidence of control group was 38.4%,and the difference was statistically significant(P<0.05).There is an interaction between the occurrence of pregnancy complications and the daily smoke exposure time during pregnancy, and both of them affect the occurrence of adverse pregnancy outcomes.ConclusionsThe passive smoking rate of pregnant women in some parts of Shandong province is low,and the main source of smoke is family and workplace.Pregnant women do not know enough about passive smoking.Passive smoking during pregnancy is a risk factor for pregnancy complications. There is an interaction between the occurrence of pregnancy complications and the daily smoke exposure time during pregnancy, and both of them affect the occurrence of adverse pregnancy outcomes. The systematic publicity of pre-pregnancy passive smoking should be enhanced to reduce the exposure of smoke during pregnancy, thereby reducing the risk of pregnancy complications and adverse pregnancy outcomes and promoting the health of future generations.
