Treatment Of Upper Limb Fracture With Unilateral External Fixator Combined With Bone No.2 Prescription:A Clinical Observation Of 15 Cases

Objective: To analyze the efficacy of the unilateral external fixator combined with Bone No.2 prescription in the treatment of upper extremity fracture in 15 cases,and to discuss its safety and superiority.Method: Retrospective collection of Orthopaedics Department of Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine between September 2014 and June 2017 used unilateral external fixator combined with Bone No.2 prescription to treat 15 cases of upper extremity fractures as a treatment group.Among them,there were 10 males and 5 females,aged15~72 years,with an average of 41.20+16.37 years.6 Cases of humeral fracture,4 cases of double fracture of the shaft of ulna and radius,5 cases of distal radius fracture.The type of fracture: 8 cases of comminuted fractures(53% of the total cases),2 cases of transverse fracture,2 cases of oblique fracture and 3 cases of spiral fracture.9 cases of open fractures(60% cases accounted for the total number of cases),according to the Gustilo classification,include 3 cases of Gustilo?type,5 cases of Gustilo?type,1case of Gustilo III type,6 cases of closed fractures.According to sex,age,fracture type and fracture site,15 patients with upper limb fracture combined with Bone No.2 prescription were randomly selected as control group.Among them,there were 8 males and 7 females,aged 20~80 years,with an average of 49.40+19.21 years.There were 8 cases of humeral fracture,3 cases of double fracture of the shaft of ulna and radius and 4 cases of distal radius fracture.The type of fracture: 6 cases of comminuted fracture(40% of the total cases),1 case of transverse fracture,3 cases of oblique fracture and 5cases of spiral fracture.5 cases of open fracture(the total number of cases were 33%),according to Gustilo classification,include 4 cases of Gustilo I type,1 case of Gustilo II type,and 10 closed fracture.The clinical healing time,complications and upper limb function of the two groups were observed by consulting the in-patient,outpatient medical records and following up regularly.Results: All patients were followed up for the shortest 10 months,the longest18 months,an average of 13.5 months.1.The healing time of the fracture in the treatment group was better than that in the control group(P=0.004 < 0.05).The healing time in the treatment group was the shortest in 3.5 weeks,the longest in 16 weeks,with an average of 13.22+3.44 weeks.The fracture healing time in the control group was the shortest 4 weeks,the longest 18 weeks,with an average of 13.22+3.44 weeks.2.In the treatment group,no complications were found in 5 cases of needle exudation,but the results of bacterial culture were all negative.In the control group,there were 2 cases of complications(13.3%),1 cases of distal radius fracture infection after operation,after dressing change and antibiotic treatment,3 months to achieve clinical healing,and remove the steel plate.One case of humeral shaft fracture developed nerve injury after operation and was given nutritional nerve and functional exercise and recovered after 3 months.3.There was significant difference in time benefit between the two groups(P < 0.01).The upper limb function score of the treatment group was better than that of the control group(P < 0.05)on the day of operation,2 weeks after operation,4 weeks after operation and the last follow-up,indicating that the upper limb function of the two groups had recovered well.The recovery of upper limb function in the treatment group was better than that in the control group on the day of operation,2 weeks,4 weeks and the last follow-up.Conclusions: 1.The two fixation methods can achieve satisfactory clinical effect in the treatment of upper limb fracture.2.Single arm external fixator is more suitable for open and comminuted fractures because of its short healing time,less complications and better functional recovery.
