The Significance Of T Wave Changes Of AvL Lead In ECG For Foreboding The Stenosis Of Coronary Artery In Patients With Stable Angina Pectoris | Posted on:2019-08-28 | Degree:Master | Type:Thesis | Country:China | Candidate:Z Z Wei | Full Text:PDF | GTID:2394330545453211 | Subject:Internal medicine (cardiovascular) | Abstract/Summary: | PDF Full Text Request | ObjectiveTo explore the importance of T wave changes in lead avL of electrocardiogram for Patients with Stable Angina PectorisResearch MethodsThis study has retrospectively analyzed a total of 256 patients(including 157 male patients and 99 female patients)who were admitted into hospital with a diagnosis of"coronary heart disease" and treated between Jan.2014 and Dec.2016 in the Qilu Hospital of Shandong University and Jinan First People’s Hospital.According to ECG results,those study subjects were divided into the three groups,namely normal ECG group,TavL change group and multi-lead ST-T changes group.All patients underwent the coronary arteriography or coronary artery CTA examination.The scores of the severity of coronary vasculopathy were carried out using improved GENSINI score and analyzed by the least significant difference(LSD method).The relationship between TavL change and each branch of coronary artery stenosis was studied in patients with stable angina pectoris.In the meantime,the levels of total cholesterol,low density lipoprotein,uric acid,homocysteine were compared in patients among groups.Results1.The degree of each branch of coronary artery stenosis was statistically analyzed using improved GENSINI score.The correlation analysis results between homocysteine and uric acid showed that two variables and the total cholesterol were correlated(low correlation).The difference of low density lipoprotein was not statistically significant.2.The influence of hypertension on the severity of coronary artery stenosis was not statistically significant.3.Conditions of each branch of coronary artery stenosis:As compared the normal ECG group with the multi-lead ST-T change group,the degree of coronary artery stenosis in the middle segment of anterior descending branch was higher(F=9.027,P=0.000)in the TavL change group.The difference was statistically significant.The least significant difference(LSD method)analysis showed that TavL change is a dependent risk factor to predict the coronary heart disease.The middle segment hemadostenosis of anterior descending branch is a dependent risk factor(P<0.05)of TavL change.The comparative results of the rest of all indicators among groups showed that the difference was not statistically significant(p>0.05).ConclusionT avL changes inversion can be used as one of the foreboding methods of the stenosis of the left anterior descending branch of the coronary artery in patients with stable angina syndrome.Research significantCurrently there are few researches regarding stable angina pectoris and pure avL lead T-wave changes,and this research will fill up this gap to remind that pure avL lead T-wave changes not paid attention to in the past is of explicit clinical significance.Via c omparing coronary artery pathological changes for the researched cases with the result of ECG in order that stable angina pectoris patients obtain greater benefits through th e simplest routine electrocardiography,and more problems are reflected through one e lectrocardiography to draw clinician’s attention thus making a guidance for treatment. | Keywords/Search Tags: | Stable angina pectoris, Electrocardiogram, stenosis of coronary artery, avL lead | PDF Full Text Request | Related items |
